About this file
This document is a set of city forms for the Mispah Parking Lot Replacement-Rebid project by the City of Leesburg, Florida, containing various compliance and certification documents for the municipal construction project. The forms include Disadvantaged Business Enterprise (DBE) Program documentation, Equal Employment Opportunity (EEO) provisions, Section 3 plan requirements, and Federal Labor Standards Provisions. These forms are designed to ensure contractor compliance with federal and local regulations regarding workforce diversity, labor standards, and subcontractor relationships for the parking lot replacement project.
The documents outline specific requirements for bidders, including commitments to DBE utilization goals (14.9% M/DBE and 6.9% W/DBE), workforce diversity tracking, and labor standards certifications. Contractors must complete detailed forms demonstrating their compliance with equal opportunity regulations, including provisions for hiring lower-income residents, maintaining non-segregated facilities, and adhering to prevailing wage requirements. The forms also include warnings about potential legal consequences for false statements and require extensive documentation of business ownership, subcontractor relationships, and organizational structure.
