About this opportunity

The Utah Department of Health and Human Services is requesting proposals for residential treatment services for two distinct programs with a proposal due date of October 16, 2026: an 8-bed male residential treatment facility for youth with problematic sexual behaviors at moderate level (DSE) and a 12-bed female residential treatment facility for high-level mental and behavioral health services (DMF).

Offerors must meet mandatory minimum requirements including state licensing and accreditation, appropriate staff credentials and training certifications, facility safety and security standards, and therapeutic programming capability. Past performance screening is strict: offerors must disclose and are eliminated from consideration if they have had DHHS or equivalent state licenses suspended, revoked, or denied for health or safety reasons within the past five years, or if any State of Utah contracts have been terminated for cause due to health or safety reasons within that period. Offerors placed on moratorium by DHHS for health or safety reasons are eliminated unless the moratorium was resolved.

Facility organization must meet strict separation requirements: facilities must operate as fully self-sufficient, independent entities with separate licenses for no more than 16 combined beds, distinct meal preparation and eating areas, non-shared direct care and supervisory staff, and separate clinical staff. Educational programming must occur within individual facilities, at separate off-site locations, in public schools open to the general community, or in community-operated schools—never in shared educational facilities exclusively serving the offeror's clients. Limited exceptions permit clinical directors and psychiatrists to serve multiple facilities only when geographic separation makes client programming combination infeasible.

Compensation is established at $424 daily for DMF services and $414 for DMF absence codes; $257 daily for DSE services and $247 for DSE absence codes. Offerors must enroll as Utah Medicaid providers and bill Medicaid directly for diagnostic and rehabilitative mental health services. Offerors may also provide additional non-Medicaid wrap services including mentoring, contracted transportation, and day group skills support where licensed.

Required submissions include completed proposal forms, organizational documentation with facility org charts, service delivery plans, staffing qualifications, compliance certifications, past performance disclosures, conflict of interest disclosures, DHHS Data Sheet, W-9, and Medicaid service assurances. Evaluation uses a standardized score sheet assessing vendor capability, clinical expertise, operational capacity, and regulatory compliance.
