About this file

This is a Mandatory Minimum Requirements Form 4 document prepared by the Utah Department of Health and Human Services (DHHS) for a residential treatment services contract opportunity. The procurement seeks to establish residential treatment programs providing services to two distinct populations: an 8-bed male program for youth with problematic sexual behaviors at a moderate level (DSE), and a 12-bed female program for high-level mental and behavioral health services (DMF). The form outlines the essential qualifications and operational standards that prospective bidders must meet to be considered for contract award, establishing baseline criteria for facility operations, staffing competencies, clinical protocols, and service delivery requirements across both residential programs.

The document serves as the mandatory minimum requirements threshold, meaning all proposals must demonstrate compliance with the specified criteria to advance in the procurement evaluation process. Bidders failing to meet any of the mandatory minimum requirements will be deemed non-responsive and excluded from further consideration. The requirements address critical elements including licensing and accreditation standards, staff credentials and training certifications, facility safety and security measures, therapeutic programming standards, and documentation and reporting capabilities necessary to serve the targeted youth populations effectively.
