The contractor must develop one Standard Operating Procedure and one standardized application for enrolling participants in the National Diabetes Prevention Program and Diabetes Self-Management Education programs, and submit quarterly activity reports documenting technical assistance provided.
Five mandatory minimum qualifications apply on a pass-fail basis: designation of a Chief Medical Officer or Executive Medical Director with an active, unrestricted clinical license and quality improvement experience; at least five years of clinical quality improvement experience with clinics and health care centers; at least five years of training, education, and technical assistance to entities implementing quality improvement projects; ability to co-chair and provide technical assistance to the Clinical sub-workgroup; and commitment to submit quarterly and annual reports. Technical evaluation uses a 1,000-point scale weighted equally (250 points each) across four criteria: resumes for at least three key project personnel, description of quality improvement experience with clinics and hospitals, description of training and technical assistance experience, and ability to facilitate meetings and chair workgroups. Vendors must achieve a minimum of 700 points to advance.
The contract term is five years beginning July 1, 2026 through June 30, 2031. The initial annual budget is $62,000; subsequent years are funded at $42,000 annually. The contractor must maintain commercial general liability insurance of at least $1,000,000 per occurrence and $3,000,000 aggregate and comply with E-Verify employment verification and all federal and state regulations.
