About this file
This is a Scope of Work document for the State of Utah Department of Health and Human Services (DHHS) establishing a clinical quality improvement technical assistance provider contract in support of the Million Hearts Coalition's cardiovascular disease reduction initiative. The contractor shall attend Million Hearts Coalition meetings, co-chair the clinical sub-group as a quality improvement subject matter expert, and provide quarterly technical assistance to two to three Local Health Departments and clinics implementing quality improvement projects. The scope requires the contractor to develop one Standard Operating Procedure and one standardized application focused on increasing enrollment in the National Diabetes Prevention Program and Diabetes Self-Management Education programs. The contract term is five years, with work beginning July 1, 2026, and spanning multiple fiscal year periods through June 30, 2031. The contractor must deliver quarterly activities reports to DHHS documenting technical assistance provided, including Local Health Department or clinic names, activity dates, personnel involved, descriptions of assistance, and any barriers encountered.
The initial contract period from July 1, 2026 to June 30, 2027 is funded at $62,000, with subsequent annual funding of $42,000 for each of the four remaining contract years. The annual budget breakdown allocates $10,000 to Million Hearts coalition participation, $32,000 to technical assistance activities, and $20,000 to Standard Operating Procedures development. All invoices shall be submitted via email to McKell Drury and Karlee Walker at DHHS with itemized descriptions of completed activities and deliverables. Performance will be measured through trainings provided, workgroup meetings co-chaired, hypertension control rates achieved, policies and workflows created, and percentage of clinical sub-workgroup participants from high-need geographic areas identified by census tract. The desired outcome is improved hypertension and diabetes control rates for Utah residents through expert clinical quality improvement support.
