The project addresses mine subsidence impacts affecting a residential structure through installation of a foundation wall anchor system to brace three concrete basement walls spanning approximately 141 linear feet at a wall height of 8 feet 8 inches. Key deliverables include furnishing and installing a ground bolt style wall anchor system with interior wall plates, exterior soil anchors, connecting steel rods, and associated hardware to stabilize lateral foundation deformations, along with mobilization, demobilization, and site restoration activities including seeding with specified grass mixtures (100 lbs Kentucky Blue Grass, 60 lbs Perennial Rye, and 40 lbs Creeping Red Fescue per acre), fertilization (100 lbs Nitrogen, 200 lbs Phosphorus, and 300 lbs Potassium per acre), and mulching at 2 tons per acre. Work shall commence within 30 calendar days of notice to proceed with substantial completion required by May 31, 2026. All pricing is established on a lump sum basis for both the foundation wall anchor system installation and mobilization/demobilization activities. The contractor must maintain comprehensive insurance coverage including Workers Compensation, Commercial General Liability with $2,000,000 general aggregate and $1,000,000 per occurrence limits, Owners and Contractor's Protective Liability at $1,000,000 combined single limit, Commercial Automobile Liability at $1,000,000 per occurrence, and Umbrella Liability coverage, with all insurers rated B+ or higher by Best's Key Rating Guide. The proposal due date is January 13, 2026, and the contract term is one year from execution date.

The procurement incorporates the Illinois Department of Transportation Standard Specifications adopted January 1, 2022, as amended by Supplemental Specifications effective January 1, 2025, and is subject to state standard terms and conditions applicable to all Illinois state agency contracts. Vendors must comply with prevailing wage requirements, maintain books and records for three years from final payment or five years if federal funds are utilized, and provide certified copies of insurance documentation before contract execution. The contractor is responsible for all insurance costs and must cooperate with owner occupancy during the entire construction period. A $15 minority contractor initiative fee will be deducted from the first payment on contracts exceeding $1,000. No incumbent vendor or specific award value has been identified in the available documentation; however, lump sum pricing for the complete foundation wall anchor installation and site restoration work is required, with the contract term extending one year from execution date.
