About this file

This document is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania under Executive Order 2021-06. The certification requires contractors and grantees to affirm compliance with various Pennsylvania state labor and workforce safety laws, including statutes related to workplace misclassification, child labor, minimum wage, prevailing wage, equal pay, employment verification, and industry-specific workforce regulations.

The certification form mandates that contractors formally attest to their adherence to 13 specific labor laws and workplace safety standards. Failure to comply carries significant penalties, including potential contract termination and a three-year prohibition from bidding on Commonwealth contracts. The document provides a formal certification section where an authorized company representative must sign, print their name and title, and confirm ongoing compliance with state labor regulations, with an explicit requirement to immediately report any changes in compliance status to the purchasing agency.
