About this file

This is a vendor affidavit document required by Hillsborough County Public Schools (HCPS) in Florida for all nongovernmental contractors executing, renewing, or extending contracts with the school district. The affidavit is mandated by Section 787.06(13) of the Florida Statutes and pertains to human trafficking compliance. An authorized company representative must sign the affidavit under penalty of perjury certifying that the contractor does not engage in coercion for labor or services. The related contract opportunity involves telecommunications services, specifically basic telephone service for HCPS, including local, IntraLATA, InterLATA, and long-distance service with PRI and/or POTS lines to be installed at schools throughout Hillsborough County in the 813-area code. Proposals must include all setup, installation, usage charges, taxes, fees, and standard monthly charges.

The affidavit requires the contractor to certify compliance with seven specific prohibitions: not using or threatening physical force; not restraining, isolating, or confining persons without lawful authority; not establishing debt through labor pledge without proper application of value; not confiscating government identification or immigration documents; not causing or threatening financial harm; not using fraud or deceit to entice persons; and not providing controlled substances for exploitation. No pricing terms, set-asides, incumbent information, funding sources, or contract term details are specified in this affidavit document.
