About this file

This is an affidavit form required by Florida Statutes Section 287.138 for entities bidding on contracts with Hillsborough County Public Schools (HCPS). The affidavit must be completed and executed by an authorized representative of any proposer or contractor submitting a bid, proposal, or quote to HCPS for telecommunications services, specifically basic telephone service including local, IntraLATA, InterLATA, and long-distance service. The services encompass PRI and POTS line installation at school locations throughout Hillsborough County in the 813-area code, along with setup, installation charges, usage charges, taxes, and standard monthly fees. No bid or proposal will be accepted without the completed and executed affidavit.

The affidavit requires proposers to certify under penalties of perjury that their legal entity does not meet any of three criteria defined in Florida Statutes: ownership by a foreign country of concern, controlling interest held by a foreign country of concern, or organization under the laws of or principal place of business in a foreign country of concern. Foreign countries of concern are defined as the People's Republic of China, the Russian Federation, the Islamic Republic of Iran, the Democratic People's Republic of Korea, the Republic of Cuba, the Venezuelan regime of Nicolas Maduro, and the Syrian Arab Republic. The definition of controlling interest includes any entity with the right to vote 25 percent or more of voting interests or entitlement to 25 percent or more of profits. An authorized representative must print their name, title, and provide a signature with date to complete the certification.
