About this file

The Unfair Business Practices Disclosure Form is a document associated with the Bernalillo County Water Authority's procurement process for the FY25 Small Diameter SAS Rehabilitation project. The form requires potential offerors to disclose any unfair business practices violations within the past three years, as defined by Section 2-376 of the Bernalillo County Procurement Ordinance. The form is part of a solicitation (B2025000012) for rehabilitating small diameter gravity pipe using trenchless methodologies at various locations in Albuquerque, New Mexico, with tasks separated into a base bid and an additive alternate based on priority and funding availability.

The document mandates that offerors certify their commitment to reporting any future unfair business practices to the Central Purchasing Office during the contract term and acknowledges that engaging in such practices may result in contract termination. The form provides space for offerors to detail any violations, including description, dates of violation and enforcement agency findings, and requires the authorized representative's signature, contact information, and company details. By executing this form, offerors agree to transparency and ethical business conduct as a prerequisite for potential contract award with the Water Authority.
