About this file
This is a Supplemental Bidder Attestation form issued by the Rhode Island Department of Administration, Division of Purchases, on behalf of the Rhode Island Department of Transportation (RIDOT) Planning Division for TRFP27006857, an on-call transit planning consultant services solicitation. The form requires bidders to certify compliance with Rhode Island General Laws 37-2-13.1(b) regarding conflicts of interest. Bidders must attest that their company and related entities, including parent corporations, subsidiaries, affiliates, and subcontractors, have no conflicts of interest with any official, officer, or agency overseeing the request for proposal, and that they did not materially participate in or provide consultation regarding the proposal's requirements, technical aspects, or formation. Additionally, bidders must confirm that the solicitation does not relate to any audit, examination, independent verification, review, or evaluation of their work, financials, or operations previously performed for the State of Rhode Island.
The attestation must be completed and signed by an authorized representative of the bidding entity, with the signatory certifying under penalty that all information provided is true and accurate to the best of their knowledge. The form clarifies that prior vendor status with the State does not automatically constitute a disqualifying conflict of interest, and that formal responses to Requests for Information do not constitute participation or consultation that would disqualify a vendor from future RFP participation. Bidders are required to provide their company name, the solicitation number and title, the date of attestation, and the authorized signatory's printed name and signature on the form.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
