About this file

This is a Supplemental Bidder Attestation form issued by the Rhode Island Department of Administration, Division of Purchases, for use in response to the DCAMM Pastore Campus Complex MS4 Stormwater Compliance solicitation. The form requires authorized signatories to certify that their company and all parent corporations, subsidiaries, affiliates, and subcontractors do not have conflicts of interest with any Rhode Island state officials, officers, or agencies involved in the request for proposal. The bidder must also attest that the company did not materially participate in or receive consultation regarding the requirements, technical aspects, or formation of the RFP. Additionally, bidders must confirm that the solicitation does not involve any audit, examination, independent verification, review, or evaluation of their work, financials, or operations previously performed for the State of Rhode Island. A pre-bid information meeting is scheduled for May 7, 2026, at 10:00 AM EST at Building 61 (Facilities Office) on the Pastore Campus located at 14 West Road in Cranston, Rhode Island.

The attestation form clarifies that being a current or past vendor to the State does not automatically create a conflict of interest requiring disqualification. Additionally, formal responses to any prior Request for Information issued by the Division of Purchases do not constitute participation or consultation in future RFPs and will not disqualify vendors from subsequent proposal submissions. The authorized signatory must provide their printed name, signature, date of attestation, and the solicitation number and title on the completed form before submission.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
