About this file

This Scope of Work document outlines requirements from the Indiana Department of Health (IDOH) for a 2-year software license agreement to support two online health applications. The vendor must provide branded iOS and Android custom applications for the National Diabetes Prevention Program (DPP) and a recognized Diabetes Self-Management Education & Support (DSMES) program, along with a branded web-based user interface. The software applications will be deployed to Indiana-based organizations to deliver these programs, with the state serving as a distribution intermediary rather than a direct program provider. All applications must be operational by the contract initiation date and include standardized features across platforms: photo meal journaling with time and portion adjustment capabilities, manual physical activity and weight tracking, automatically updated weekly or monthly video lessons, PDF lesson viewing functionality, community support features enabling peer interaction and commenting, and private messaging with coaches including access to customized video and PDF content.

The Scope of Work does not specify pricing terms, contract award dates, bidder meeting information, or funding sources and amounts. No details regarding set-asides, incumbent vendors, or current system environment are provided in this document. The procurement is limited to software licensing requirements without addressing budget parameters, payment structures, or renewal options beyond the initial 2-year term.
