Responding firms must confirm eligibility requirements by attesting to their agreement with the State/Designer Basic Services Agreement, their ability to provide a Certificate of Insurance upon award, submission of a signed Affidavit of Non-collusion, completion of a Workforce Certificate if applicable, and disclosure of any foreign outsourcing involvement. All proposals must be signed by an authorized member or officer of the responding firm and must include current information regarding the signatory's name, title, professional registration number, corporate registration status in Minnesota, MN Tax ID Number, Federal Tax ID Number, and MN Vendor Number. The form explicitly states that only the provided forms should be used and that responding firms should refer to the Scoring Criteria section of the RFP for specific evaluation standards.
