About this file
This is a Respondent Information Form (Attachment A) for Request For Offer DIR-CPO-TXA-610 issued by the State of Texas Department of Information Resources for Texas Agency Network Wireless, Pagers, and Satellite Services, and Related Hardware (TEX-AN-WPS). The form serves as the primary vehicle for collecting essential respondent information and certifications required for participation in this procurement. Respondents must complete all sections of the form, including entity information, contact details for RFO inquiries, negotiations, and contractual binding authority, along with comprehensive conflict of interest disclosures for both respondent and subcontractor personnel. The form requires identification of any current or former state employees assigned to work 20% or more of their time on any resulting service agreement and disclosure of respondent personnel related within two degrees of consanguinity to current or former state employees. Additionally, respondents must disclose information regarding any former executive heads of state agencies employed by the respondent, as required under Texas Government Code Section 669.003.
The form includes mandatory certification statements covering financial stability requirements, prohibition from federal and state debarment lists, compliance with antitrust laws, and restrictions on improper payments or gifts to public servants. Respondents must certify they are not listed on the prohibited respondents list under Executive Order 13224, are not owned or controlled by foreign adversaries, and meet Texas resident bidder qualifications if claiming a Texas principal place of business. All certifications must be signed by an officer or agent with authority to contractually bind the respondent, and the form explicitly states that these representations will be incorporated into any resulting contract. The Texas Comptroller of Public Accounts must identify respondents as "in good standing" and authorized to conduct business with the state, and respondents must remain current on all tax obligations throughout the procurement process and contract term.
