About this file

The document is Revised Attachment A: Administrative Requirements for ITN No. DMS-24/25-232, a Group Term Life Insurance contract issued by the Florida Department of Management Services (DMS), Division of State Group Insurance. The document outlines comprehensive administrative requirements for the contractor, with an implementation date of January 1, 2026. Key implementation milestones include establishing a Department-approved website by August 1, 2025, functioning toll-free phone lines, participating in Fall 2025 Open Enrollment benefit fairs, and conducting weekly implementation status meetings. The contractor must provide a final implementation plan within ten business days of the contract's effective date, detailing all steps necessary to begin full performance, including background checks, communication material development, and eligibility file testing.

The administrative requirements cover extensive operational aspects, including customer service operations, with mandates for 24/7 support, toll-free phone lines answered within 30 seconds, and a call abandonment rate not exceeding 3%. The contractor must maintain a participant website, process medical underwriting within 10 business days, provide notice of approved coverage within 5 business days, and create beneficiary designation forms. Reporting obligations are significant, including monthly enrollment discrepancy reports, quarterly claims experience and performance guarantee reports, and annual participant satisfaction surveys. The document emphasizes strict compliance with confidentiality standards, particularly the Gramm-Leach-Bliley Act, and requires the contractor to maintain records for ten years after claim disposition or contract expiration. Audit provisions are robust, with requirements for quarterly internal audits, immediate discrepancy reporting, and comprehensive cooperation with Department-initiated audits.
