About this file

This is a Past Performance Questionnaire (PPQ) template designed to collect references from clients regarding contractor performance on previous contracts with the National Institute of Standards and Technology (NIST). The form solicits contractor information including firm name, address, contact details, and identification of whether the respondent is a prime or subcontractor, along with corresponding client information.

The questionnaire requests detailed information about machined or fabricated parts and components previously purchased, including quantity, purchase date, and whether products arrived in operating condition meeting all technical requirements. Contractors must document delivery performance against schedule and explain any delays. The form includes a standardized performance rating scale with five levels—Excellent, Very Good, Satisfactory, Marginal, and Unsatisfactory—with detailed criteria for each rating category. The questionnaire must be completed and signed by the client, with instructions indicating that clients should submit completed forms directly to the offeror, though submission may also be made to the contracting officer and secondary points of contact. Offerors are instructed to submit completed questionnaires with their quotation and may reuse the form for future NIST solicitations as applicable.
