About this file
Summary
This is an Architectural Description Document (ADD) for the South Carolina Department of Health and Human Services (SCDHHS) Member Management Replacement Program (MMRP) Curam Global Income Support (CGIS) Project, specifically addressing the CGIS12 Buy-In Interface. The document details the comprehensive technical and functional design for a Medicare buy-in interface system that processes requests between SCDHHS and the Centers for Medicare & Medicaid Services (CMS). The system handles two primary Medicare categories—Part A (hospital care) and Part B (physician and outpatient services)—and manages the complete lifecycle of buy-in transactions including accretion requests (adding beneficiaries), deletion requests (removing beneficiaries), simultaneous accretion/deletion requests, and update requests. The interface processes daily outbound files to CMS containing beneficiary information, receives inbound response files with Record Identification Codes (RIC A through RIC F) indicating acceptance, rejection, or adjustments, and generates monthly billing files for state payment to the Centers for Medicare & Medicaid Services. The document version 3.0 was approved on April 11, 2021, following extensive review cycles and change requests incorporated from December 2018 through November 2020.
The technical design encompasses multiple integrated components including Curam batch processing (outbound chunker and streamer processes), Enterprise Service Bus (ESB) file handling and transformation between EBCDIC and ASCII formats using JRecord APIs, database persistence across multiple tables, and comprehensive error handling with exception reporting. The system maintains data integrity through duplicate record detection, validation against CMS file formats, and a deduplication logic preventing duplicate outbound requests for the same beneficiary with identical transaction types and effective dates. The interface includes user-facing functionality allowing authorized SCDHHS personnel to manually create buy-in requests, review pending approvals, view transaction histories, and manage exception records. The system generates multiple control and exception reports tracking record counts, processing status, validation failures, and financial transactions, with all validated records retained for three years. Email notifications alert designated listservs regarding batch execution completion, file transmission status, and pending approvals requiring action within specified timeframes.

1 Introduction
1.1 Background
The Buy-In interface is a process through which the state Medicaid program accepts responsibility to pay Medicare Part-A and Part-B premiums for qualified Medicaid recipients.
Medicare Part A covers hospital care. Most individuals are eligible for free Medicare Part A, however there are individuals who are not eligible for free Part A. South Carolina is a Group
Premium Payer state and as a Group Premium Payer state, before we can begin paying the
Medicare Part A premium, the individual must have filed an application for actual or conditional
Medicare Part A with SSA. To conditionally enroll, the individual must indicate to SSA that they wish to enroll in Medicare Part A only if the State will pay the Part A premium. Persons who conditionally enroll do not become eligible for Part A until the State submits a Buy-In request to
CMS and that request is accepted.
Medicare Part B covers doctor’s services, outpatient care, and other medically necessary medical services. SC Medicaid covers the Part B premium when the beneficiary is eligible for full Medicaid benefits and Medicare Part B. It also covers the Part B premium when the person is eligible for
Specified Low Income Medicare Beneficiaries (SLMB) or Qualifying Individuals (QI) payment categories.
1.2 Purpose
The purpose of this document is to detail screen flows and changes needed in Cúram Global
Income Support (CGIS) to implement SC MMRP configurations, extensions and customizations to implement the MMRP CGIS solution only.
The intended audiences for this document are:
• MMRP Subject Matter Experts (SMEs)
• MMRP Cúram – Business Analysts
• MMRP Cúram – Architecture team
• MMRP Cúram – Solution Developers
• MMRP Cúram – Testing Team
1.3 Scope
The scope of this project is to perform the following:
State to CMS - Outbound files
The system identifies all Buy-In Part A and B eligibles, ineligibles and changes in information.
The system will generate requests for a Buy-In accretion, Buy-In deletion, Buy-In Updation and simultaneous accretion/deletion to CMS. The system will also have the capability for specific user roles to create manual outbound requests. Separate Part A and Part B Buy-In outbound files are sent to CMS.
CMS to State Inbound files
CMS will return a response for every Buy-In request made by the state. CMS sends separate Part-
A and Part-B inbound files to the state. Each record received will have a Record Identification
Code (RIC). The system will validate and process the information received. If there are no discrepancies, the member’s Buy-In records are created/updated where necessary. If there are discrepancies, tasks are created and assigned to work queues for manual intervention.
CMS to State Billing files
CMS sends a separate billing file for Part-A and Part-B every month. Each record received is processed based on the transaction code and the Premium Amount is updated where necessary. If there are discrepancies, tasks are created and assigned to work queues for manual intervention.
State to MMIS Claim file
The state will send a claim file to MMIS containing Part-A and Part-B billing record at the end of every month.
