ATTACHMENT 6

Instructions for Submission of Proposal

(Request for Proposal - P15PS00380)

PROPOSALS WILL ONLY BE ACCEPTED IF SUBMITTED ELECTRONICALLY BY E-MAIL.

1. Documents/Information To Be Submitted With Proposal: At a minimum, the following documents/information shall be completed/provided and submitted with your proposal.

Clauses and Provisions indicated below which are incorporated into the solicitation by reference rather than full text, can be accessed online, in full text, at:

a. Offeror should submit all documents in .pdf format to the greatest extent practical.

b. Standard Form 1442, Solicitation, Offer, Award Form: Complete Blocks 14 through 20C

c. Attachment 4, Offeror’s Proposed Price Schedule.

d. Section I, Clause 52.223-3 Alternate I, Hazardous Material Identification and Material

Safety Data (if applicable)

e. Section I, Clause 52.225-9, Buy American Act-Construction Materials (if applicable)

f. Section K, Provision 52.204-8, Annual Representations and Certifications Offeror is to indicate which of the two paragraphs in 52.204-8 apply --- Either (b)(2)(i), or

(b)(2)(ii).

- If paragraph (b)(2)(i) applies, submit first page of your firm’s Representations and

Certifications from the www.sam.gov website

- If paragraph (b)(2)(ii) applies, offeror is to have completed all individual representations and certifications and include them with his/her proposal.

g. Section K, Provision 1452.215-71 – Use and Disclosure of Proposal Information –

Department of the Interior (if applicable)

h. Attachment 5, Financial and Technical Capability Submittal.

i. Section M, Evaluation Factors for Award - Provide the information and documents requested in Paragraphs (b)(1) and (b)(2).

j. Any other information or documents requested by the solicitation that are applicable to the offeror’s proposal. See SECTION K, Provision 52.236-28, and SECTION L, Provision 52.215-1.

k. Acknowledgment of all Amendments issued against this solicitation.

2. Point of Contact: The Contracting Officer for this solicitation is Kathryn M. Logsdon, at telephone number (217) 391-3225, or by e-mail at kathryn_logsdon@nps.gov.

3. Amendments to Solicitation: It is the responsibility of the offerors to monitor the FBO and

FedConnect websites to download and acknowledge all amendments issued against the solicitation.

4. System for Award Management (SAM) and Online Representations and Certifications: All offerors are to have a Dun & Bradstreet number, be registered and active in the System for Award Management (SAM), have a NAICS Code of 238220 listed in their SAM profile, and have completed their Online Representations and Certifications in order to be awarded a contract; therefore, if you have not yet completed these requirements, it is strongly advised that you do so at this time. Failure on the part of the successful offeror to have these requirements fulfilled by the time the Contracting Officer makes award may result in award of the contract being made to the next otherwise successful offeror. 5. Explanation to Offerors:

a. Offerors may request an explanation or interpretation of the solicitation, or its attachments by contacting the Point of Contact named in paragraph 2 above.

b. When sending a question(s) via e-mail, prospective offerors are requested to include the solicitation number and contract title in the subject line of the e-mail and the Company’s full name and address within the body of the message. Questions are to be submitted no later than 4:30 p.m. central time on Thurdsay, June 11, 2015.

6. Delivery of Proposals:

a. Proposal packages are to be received by the Contracting Officer no later than Friday, June 19, 2015 at 2:00 p.m. central time. Failure to submit your proposal package so that it is received by this time and date may result in the proposal not being considered.

b. Proposals are to be e-mailed to: kathryn_logsdon@nps.gov

Facsimile proposals will not be considered.

7. Notice of Pre-Award Survey:

a. No award shall be made unless the Contracting Officer can make an affirmative determination of responsibility of the prospective contractor as per the standards identified in FAR Subpart 9.1. Offerors are advised that this is accomplished through a pre-award survey which may require the prospective contractor to provide additional information or documentation as requested by the Contracting Officer. Areas of interest on this survey may include, but are not limited to the following, with regard to the prospective contractor:

(1) Have adequate financial resources to perform the contract, or the ability to obtain them.

(2) Be able to comply with the required delivery date, taking into consideration all existing commercial and governmental business commitments.

(3) Have a satisfactory performance record.

(4) Have a satisfactory record of integrity and business ethics.

(5) Have the necessary organization, experience, accounting and operational controls, and technical skills, or the ability to obtain them, including, as appropriate, such elements as production control procedures, property control systems, quality assurance measures, and safety programs applicable to materials to be produced or services to be performed by the prospective contractor and subcontractors.

(6) Have the necessary production, construction, and technical equipment and facilities, or the ability to obtain them.

(7) Be otherwise qualified and eligible to receive an award under applicable laws and regulations.

b. Offerors are advised that accomplishment of this survey is a part of the determination of responsibility process and is not to be construed that a request to the prospective contractor for additional information and documents is an indication that he/she will receive the resultant award.

8. Compliance: By submitting a proposal, the prospective contractor agrees to comply with all terms and conditions as set forth in the solicitation and amendments issued thereto.
