About this file

This is an insurance requirements document (Form OS-52) issued by the Pennsylvania Department of Transportation for state and local contracts, specifically applicable to the Safety Service Patrol contract in York County. The contract requires the awarded contractor to provide expeditious removal of disabled or accident vehicles and small non-hazardous debris from roadways to reduce traffic delays and congestion. The contractor must purchase and maintain all required insurance coverage at its own expense prior to commencing work or entering Department of Transportation property, with insurance remaining in effect throughout the entire contract term and any renewals or extensions.

The contractor must obtain workers' compensation insurance as required by Pennsylvania law and comprehensive general liability insurance with minimum coverage of $250,000 per person and $1,000,000 per occurrence for both bodily injury (including death) and property damage. The policies must be occurrence-based and name the Commonwealth or Department as an additional insured party, with provisions requiring thirty days' prior written notice of cancellation or changes (fifteen days for non-payment of premiums). The contractor must provide current certificates of insurance to the Department prior to work commencement and must notify the Department immediately of any insurance cancellation. No work or entry onto Department property may occur during any period when insurance coverage is not maintained. If required information cannot be displayed on insurance certificates alone, relevant policy documents demonstrating additional insured status and cancellation notice provisions must accompany the certificates.
