About this file
This document is a compilation of supporting forms and questionnaires for a New York State Department of Transportation (NYSDOT) purchasing solicitation for potential contract opportunities. The file contains multiple certification forms including Procurement Lobby Act, Executive Order 177, Executive Order 16, Vendor Assurance of No Conflict of Interest, Manufacturer's Certification, and Vendor Responsibility Questionnaire forms. These documents are required submissions for bidders/vendors seeking to participate in NYSDOT procurement opportunities, with specific requirements related to business operations, non-discrimination policies, conflict of interest disclosures, and compliance with state regulations.
The forms address critical vendor compliance areas, including verification of business operations (particularly regarding business activities in Russia), human rights law adherence, potential conflicts of interest, and vendor responsibility. Vendors must complete various certifications that cover topics such as institutional policies against discrimination, potential conflicts with existing state contracts, and disclosures of prior non-responsibility determinations. The documentation also includes tax-related forms (ST-220), registration requirements for public work projects, and specific attestations about the vendor's current business practices and ethical standards. Vendors are encouraged to complete these forms electronically through state systems like the Office of the State Comptroller's Vendor Management System to expedite review and approval processes.
