About this file

This document is a No Conflict of Interest Certification Form issued by the New York State Department of Transportation's Administrative Services Division - Purchasing. The form requires vendors, contractors, subcontractors, and consultants to attest that their proposed services will not create conflicts of interest with existing state contracts or detract from other state projects. The certification mandates that vendors confirm their performance will not violate existing agreements, compromise their obligations to the state, or potentially harm state interests.

The certification form includes eight specific attestations covering areas such as maintaining contract integrity, avoiding resource diversion between state projects, ensuring no ethical violations involving former state or company employees, and prohibiting offering gifts that could influence state employees. The document serves as a formal declaration of ethical conduct and transparency, providing the New York State Department of Transportation with assurances that potential vendors will act in the state's best interests during procurement processes. Vendors must sign and date the form, acknowledging their understanding and agreement to comply with the state's conflict of interest procedures.
