Item No: 1144 Report Number: GM3071249-B - Damage Survey (Class) Report Date: 03-Feb-2016 Opened In Report: Guam Item Name: Fuel Oil Settling Tank - C Found: It was reported by the Port Engineer and Chief Engineer that an area on the bulkhead below the F/O settling tank was found slightly wet with fuel oil. It appears that the source of the fuel is from a pin hole in the fillet weld on the settling tank on BHD 60. Precautions have been made to avoid any hot work in this area and monitor for potential contamination of the bilge.
Recommendation: Repair pin-hole to the satisfaction of the attending surveyor. The Chief Engineer is to monitor the area and notify ABS immediately if the conditions worsens.
Full Rectification: At centerline, in-way of the pinhole leak, bulkhead 60 fillet weld was removed from both engineroom side and fuel oil settling tank, approximately eighteen inches in length. The joint was then prepped for a full penetration weld with a single bevel on the bottom of the fuel oil settling tank; Engineroom side of weld with bevel was welded first and then weld was back gouged from inside the fuel oil settling tank. The fuel oil settling tank was then welded internally. The final welds were visually and dye penetration examined and vacuum box tested; No leaks were noted and repair is considered satisfactory at this time.
