BLOCK 4: The Contractor shall deliver an initial Impact Assessment Report detailing estimated TAT following completion of the initial inspection/assessment.

The Contractor shall utilize the established Delay Notification Process. This process uses the DNF to document delays during modification or maintenance activity. The DNF details changes in the delivery schedule (TAT) associated with excusable delays as outlined in FAR 52.249-14, including contractor caused delays, duration-based delays, and hours-based delays as calculated per Attachment 7.

The Delay Notification shall include: the relevant contractor Notification Form (DNF) and a technical narrative describing the causes of the impact with relevant backup documentation as follows:

-Funding Related Delays: Contract Letter submitted in accordance with FAR 52.249-14 to request additional funding and any subsequent letters related to work delays or stoppages.

Late Contract Changes - Contract Letters and Contract Modifications to document changes or late award of additional scope.

-Contractor Caused Delays -Contractor will provide relevant documentation to explain the reason for delay. No TAT relief will be granted for these delays.

BLOCK 12: First submission will be at within 10 days of discovering delays to the original established TAT as determined by the Initial Impact Assessment, of the aircraft. Extensions may be requested pending completion of the AEPD Inspections.

BLOCK 13: Subsequent submissions will be as required, but not to exceed monthly, with a Final Impact Assessment submitted within 30 Days of DD250 of the aircraft. The final impact assessment shall include Over and Above related delays.

The contractor will provide all over and above completed documented in CDRL A00Q, the Over and Above Status report. TAT extensions shall be granted based upon the total number of O&A s negotiated. The final impact assessment will also include a summary of all duration based delays.

BLOCK 4: Deliver in a Microsoft Office format (2010 or later); such as, Excel or Access.

The Contractor shall provide Contractor Monthly Status Report submittals per DID DI-MGMT-80368A with the following exceptions; paragraphs 3.2.1, 3.2.2 and 3.2.3 are not applicable.

Report shall include:

• Tasks completed since last report.

• New tasks started since last report.

• Total projected man hours for each uncompleted task on report

• Total man hours performed on completed tasks

• Estimated completion dates for work in progress

BLOCK 8: Government comments will be provided to the contractor NLT 10 days after receipt of contractor submittal. Revised contractor submittal shall be provided within 10 days after receipt of Government comments or in the next scheduled submission.

BLOCK 12: First submission required 30 calendar days from receipt of first Task Order

Blocks 10 - 13: Prepare the Sustainment Functional Cost Hour Report (SFCHR) in accordance with DI-FNCL-81992 (or the most recently approved version) and the OSD Deputy Director, Cost Assessment (DDCA)-approved contract CSDR Plan.

The DID, DI-FNCL-81992, is available from the Defense Cost and Resource Center (DCARC) website at http://cade.osd.mil/csdr/FormsReporting.aspx#DIDs. The CSDR Plan is included as a contract attachment 18.

Contractors shall be required to submit the Contractor Cost Data Reports (CCDRs) at frequencies specified in the OSD DDCA-approved CSDR plan and in the contract. The contract CSDR plan uses the event field as the driver for the submission of the reports, not the "as of date." If the event slips, the contractor must notify the Government Program Office that a date change is needed. It is the responsibility of the Government Program Office to submit a request for change in the event-driven date for reporting through the CSDR Submit- Review system for DCARC approval before the date reflected in the OSD DDCA-approved CSDR Plan.

All CCDRs shall be submitted electronically using the CSDR Submit-Review System. The required form and file type for each CCDR is specified in its Data Item Description (DID). Data submitters must register through the DCARC website and possess a DoD-approved ECA digital certificate or DoD-issued CAC to obtain a DCARC Portal account and be authorized to upload CSDR content. Users can obtain access by submitting user information about themselves and their organizations to the DCARC Portal and requesting a CSDR submitter user role. After the registration information has been verified, the DCARC shall authorize the user account and requested roles. All DCARC Portal accounts need to be renewed at least annually.

Subcontractor Reporting: Prime contractors are responsible for flowing down CSDR requirements contained in their prime contracts to all subcontractors who meet the reporting thresholds specified in the DoDI 5000.02, or as required by the CWIPT. This includes requiring subcontractors to electronically report directly to the DCARC using the CSDR Submit-Review System. The prime contractor shall be required to work with the CWIPT and all appropriate subcontractors to prepare separate subcontract CSDR plans for submission to the DCARC for DDCA approval.
