Addendum 52.212-1 Instructions To Offerors
1. Summary: This is a request for quotation. Responses to this solicitation are considered quotations and not offer. In those instances where the words offer, offeror, and proposal are used, the reader should understand that the use of such words does not change the nature of this request for quotation. The resulting award will be a Firm Fixed Price Contract for Audio Visual Preventative Maintenance, Repair and Installation for the School of Infantry West located aboard Marine Corps Base Camp Pendleton, California.
2. Submission of Quotations: Quotations must be returned to the address listed below no later than the date and time specified in block #8, page #1 Pacific Standard Time (PST). Quotations may be hand carried, emailed, or submitted by way of commercial courier service (FedEx, UPS, etc.). Faxed quotations will not be accepted. Do not attempt to send the quotation via the US Postal Service since the US Postal Service does not deliver to the building. When submitting via Email, offerors should limit attachment size to 2MB. If necessary, multiple Emails should be submitted. In all cases, offerors should obtain a proof of delivery/proof of receipt of submission. If the offeror does not receive a proof of receipt, offeror should assume his/her submission did not reach the individual below. The Government reserves the right to make an award solely on initial quotes received.
Offerors bear the burden of ensuring that all portions of the offer (and any authorized amendments) reach the designated office on or before the deadline specified in the solicitation. Quotations shall be delivered to:

3. Contract Authority: The Government will solicit and award this contract using Simplified Acquisition Procedures, in accordance with FAR part 13 and FAR part 12. As a result of this authority, certain requirements for soliciting, awarding, and notifying quoters are streamlined. In accordance with FAR 12.102(c), when a policy in another part is inconsistent with FAR Part 12, Part 12 shall take precedence.
4. System for Award Management (SAM): Contractors must be currently registered in the System for award Management (SAM) to be eligible for award. For information and registration go to http://www.sam.gov. Note: Not required to complete Provision 52.212-3 if registered in http://www.sam.gov.
5. Period of Acceptance of Offerors: The pricing terms and conditions of the quotations should be valid for 90 days unless stated otherwise in the quote.
6. Site Visit: A site visit has been arranged for the following date, time and location. It is highly recommended that bidders attend, as it has been deemed necessary for adequate quote preparation. Site Visit details are as follows:

7. Questions: Questions shall be submitted by email no later than 24 June 2015 by 5:00 P.M. Pacific Standard Time (PST) to the email addresses listed below. No phone questions will be accepted. The deadline is necessary to ensure timely award and the Government may, in its sole discretion, choose not to respond to questions received after the deadline. All questions will be reviewed and responded to by an amendment to the solicitation, if required.
8. Complete Quote: The Government will award a Firm Fixed Price Contract resulting from this solicitation to the responsible quoter whose quotation conforming to the solicitation represents the Lowest Price Technically Acceptable (LPTA). This RFQ is for Audio Visual Preventative Maintenance. A complete quote must be received for consideration. A complete quote is as follows: (a) - (f):
a) Price: Completion of Contract Line Item Numbers (CLIN) - Insert unit prices for each CLIN, reference pages 4-28.
b) Solicitation Provisions: Completion of all solicitation provisions, as required. If information is “None” or “Not Applicable” state explicitly. Note: Not required to complete Provision 52.212-3 if registered in http://www.sam.gov.
c) Certify Submission: Signed Solicitation and any solicitation amendments, if applicable.
d) Environmentally Preferable Submission (Not-to-Exceed 1 page): Describe how your products/services support DOD Sustainable Acquisition Goals. This includes products that meet the following criteria: energy-efficient and water-efficient, bio-based, environmentally preferable, non-ozone depleting, are made with recovered materials, or use renewable energy technologies. If products/services are not sustainable, but your company engages in sustainable practices, please describe them. See FAR Part 23. An offer which more effectively advances sustainable acquisition may be determined to be a better value.
e) Technical Capability (Not-to-Exceed 3 pages): Describe the Contractor’s approach and plan in installing, maintaining and repairing audio visual systems meeting all requirements as provided in the Statement of Work. List any certifications, if applicable and address contractor’s employees experience with maintaining and repairing audio visual systems.
f) Past Performance: Provide a minimum of three (3) recent and relevant references. Recent is defined within the last 5 years. Relevant is defined as providing audio visual installation, maintenance and repair as provided in the Statement of Work. Each Reference should contain the following information:
1. Name
2. Title
3. Company/Business
4. Address
5. Email/Phone number
6. Dollar Value of Service Period
7. Description of Service Period
8. Dates of Provided Service Incomplete quotes may not be considered.
