M00263-15-R-0001 Quality Assurance Surveillance Plan
QUALITY ASSURANCE SURVEILLANCE PLAN
FOR
ALTERATIONS OF
US MARINE CORPS UNIFORMS
USMC MCRD/ERR
PARRIS ISLAND SC
ATTACHMENT 8
INTRODUCTION
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate contractor actions while implementing the performance work statement. It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on the Performance Requirements Summary (PRS) in the alterations contract.
The QASP provides a systematic method to evaluate the alteration services the contractor is required to furnish. The QASP is based on the premise the Government desires to maintain high quality standards in fitting, tailoring and altering Marine Corps uniforms.
A service contract to provide alterations is the best means of achieving that objective.
The contractor, not the Government, is responsible for management and quality control actions to meet the terms of the contract. The role of the Government is quality assurance to ensure contract standards are achieved.
In this contract the quality control program is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure the quality control program approved at the beginning of the contract provides the measures needed to lead the contractor to success.
Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a strong quality assurance program.
Performance Requirements. The performance requirements set forth in this section correspond to the PWS paragraphs delineated in the table, Performance Requirements Summary (PRS):

Surveillance: Quality inspections will be accomplished through one of the following techniques:
Random Sampling: With random sampling, services are sampled to determine if the level of performance is acceptable. A random sampling of 10% shall be performed by the Contracting Officer Representative (COR).
Periodic Inspection, Judgmental Inspection or Planned Sampling: This method, sometimes called "planned sampling, " consists of the evaluation of tasks selected on other than a 100% or random basis.
Standard. The Contractor will be evaluated on the quality of the alterations performed and adherence to Federal Standard 751 and conformance to the latest TM 10120-15/1B, Marine Corps Uniform Fitting and Alteration Manual. The maximum acceptable defect, in any given month, to include items that require rework shall not exceed 3%.
The Contractor will be evaluated on the quality of their technical ability to identify and preclude problems, or corrective actions; and effective use of their resources to meet the military uniform regulations and delivery of uniform schedules.
Procedures. The contractor shall establish and maintain a Quality Control Program that will ensure that the requirements of the contract are performed as specified. A written Quality Control Program shall be submitted as a part of contractor’s proposal as set forth in Section L, Instructions to Offerors. The Program shall include:
1. A quality control inspection system covering all contract services. The system must specify the areas to be inspected on either a scheduled or unscheduled basis, and how inspections are to be conducted.
2. The name(s) and qualifications of the individual(s) tasked to perform the quality control inspections, and the extent of their authority.
3. A method for identifying deficiencies in the quality of service performed and procedures to be taken to implement corrective action before the level of performance becomes unsatisfactory.
4. As part of the contractor’s Quality Control Program, the contractor shall, at a minimum, perform a monthly inspection. Results of the inspection shall be recorded on an inspection sheet and certified by authorized contractor personnel tasked to perform the inspection.
A copy of each report shall be submitted to the COR, on or before the fifth working day of the month following the inspection. All quality control inspection reports and any corrective actions required shall be maintained by the contractor throughout the term of the contract.
5. After each recruit’s uniform has been inspected by a fitter and marked by the tailor, the recruit will report to the board man who will order the alterations and record the marking(s) onto the board sheet by schedule line item number. The Government will total the board sheets daily, weekly, and monthly by line item number. The line item totals on each board sheet represent the orders placed for each garment. The contractor will be provided with copies of the board sheets. The recorded totals will be verified against the contractor’s monthly invoice.
6. The uniforms shall be returned to the COR after they have been altered. A random sampling of 10% shall be performed by the COR. The altered uniform shall be compared with the alteration requirement on the board sheet for that particular clothing appointment. Quality inspections shall be performed to ensure that all alterations are performed in accordance with Marine Corps Uniform Fitting and Alterations Manual, TM 10120-115/1B. The minimum acceptable quality level shall be 97%. For the purpose of inspection, the maximum acceptable defect, in any given month, to include items that require rework, shall not exceed 3%.
7. Alterations that were properly made, but do not fit correctly because of weight gain, weight loss, etc. will be re-altered. The COR or government representatives will make the determination to re-alter. The contractor shall be reimbursed at the contract rate for that particular alteration. Total re-alteration cost shall not exceed 20% of the total cost for the garment without prior approval from the COR.
Performance Evaluations Meetings. The contractor shall meet with the Government representative(s) weekly during the first three (3) months of the contract. Meetings shall be held as often as necessary thereafter, usually once a month, to resolve problems and to promote satisfactory contractor performance. The meetings shall be recorded by a Government representative, and signed by the contractor. If the contractor disputes the minutes as recorded by the Government, the contractor shall submit disputes to the Contracting Officer in writing within three (3) working days of the meeting.
Nonperformance or unsatisfactory performance. The Government may withhold payment for any items in which service has been considered nonconforming, as specified below:
1. A service task may be composed of several line items. Contractor shall not invoice the Government for any line item until the total alterations required for that garment are complete.
Contractor shall not invoice for a single line item when the service task is partially completed.
2. In the case of nonconforming and/or unsatisfactory performance, the COR shall immediately notify the contractor of the defect and allow the contractor to remedy or cure the unsatisfactory performance.
The contractor shall re-alter or cure the defect or nonperformance within one business day from notification by the COR.
A record of any unsatisfactory performance from the inspector’s daily reports shall be maintained by the COR. When contractor performance exceeds the allowable 3% defect level, the COR shall notify the Contracting Officer. The Contracting Officer may seek any of the following remedies:
(1) Allow the contractor to correct all deficiencies and perform the work within one business day as specified above. The Contractor performs the work at no additional cost to the Government.
(2) Withhold a portion of the contractor’s invoice equal to the total amount of the list prices of any nonconforming items.
(3) Report poor performance to the CPARS should the level of performance continue to deteriorate, issue a cure notice or not exercise the next option period.
