SEED Rural Group 1 Construction Services
Monthly Report Template
Section J.9 – Monthly Report Table of Contents
Section 1: General
1. Summary: Summary of key work completed, and milestones accomplished. The summary shall include a table of each site’s status and progress since last month.
2. Schedule: Updated project schedule with actual progress compared with the baseline established at the beginning of the project. USAID will specify the format and program to be used, such as MS Project. A baseline schedule must be established and compared with actual and updated schedule each month.
3. Cost: Updated projected milestone payment summary with anticipated dates for each payment.
The projected milestone payments should be depicted in Table format with corresponding s‐ curve. A baseline cost projection must be maintained and compare with the actual and updated projections each month.
4. Lessons Learned: Summarize lessons learned over the past month.
Section 2: Site Specific
1. Schedule: Site specific schedule and upcoming activities.
2. Scope: Highlight major work completed over the past month and planned work for the following month. Include any updates, revisions or impacts to QCP, CRMP, Health & Safety, environmental compliance and other plans. Track any changes made to the project scope. Include updates and status on EMMP implementation.
3. Constraints: Summary of all the problems encountered, corrective action proposed or taken, responsible parties and a timeframe for resolution.
4. Issues Log: Include a log for community raised issues and proposed actions, as well as a defects tracker.
5. EMMP Checklist: Summarize actions taken on the EMMP.
6. Photographs: Each building at each site shall be photographed to clearly demonstrate the status of each structure at month end.
