Question 1: Project Descriptions are required in multiple locations (Tabs 1, 3, 4, 7, 10; and the SF330) each location has varying requirements, none of which appear to clearly coincide with “Part IV Projects” page xii. In which tab(s) – or in what manner - is the form entitled “Part IV Projects” to be used as part of the Offeror’s response?
ANSWER: Part IV is asking for projects similar in scope, size and complexity; independent of all other project descriptions and factors. Part IV is for evaluation purposes.
Question 2: Page 10 Factor 2: A Project Manager is listed under item e. but is not mentioned in items #1 – 8 for key personnel. Should we include a project manager?
ANSWER: Please exclude project manager from this factor.
Question 3: Page 10 Factor 2: For #6 Security/Blast Consultant, is this one position or two? Should we include a person for security and another for blast? Can you please clarify security as in security cameras and systems or AT/FP? Can you please clarify the security/blast consultant’s role?
ANSWER: Security and blast consultants are separate consultants. Security covers and relocation or modernization of security equipment (cameras, x-ray machines, magnetometers ect.). Blast consultants are related to the window removal and replacement.
Question 4: Page 10 Factor 2: Can one person hold more than one role/position or should there be only one person per role/position for this project? (i.e. Lead Designer as Architect, Electrical Engineer as LEED Consultant)
ANSWER: However the offeror determines what would best suit the project and meet the requirements.
Question 5: Page 10 Factor 2: Should the resumes be submitted in a particular format – offeror’s choosing or is there a particular format GSA is requesting?
ANSWER: Resumes can be submitted in a format that meets the requirements of the RFQ and that specific factor. The format is not important; however the pertinent information must be included.
Question 6: Pages 10 – 11 Factor 3: Is GSA requesting only 3 projects or a minimum of 3 projects?
ANSWER: At least three projects, which means minimum Question 7: Page xii – xiii: GSA has provided a Part IV Projects sheet for the DBT, Construction and/or Design. Is this form required in addition to the information already provided for the general contractor/design builder, lead designer, and design team? If so, does this form count towards the final page count, and for which projects is this Part IV form required for? Should we fill out a Part IV form for the projects under Factor 3 on page 7, the projects listed for the lead designer, the projects listed under Factor 3 on page 10, and the projects included in the SF330? Where should the Part IV forms be included, under which tab?
ANSWER: Part IV forms should be submitted for projects of similar scope, size and complexity. It does count as the final page count. The Government will accept the project sheets with Tabs 3 & 10, or as a separate tab.
Question 8: Pages 8 – 9: Factor 1 requests no more than 5 projects for the lead designer. Then Factor 4 requests up to 3 projects. The requested information for the projects seems to be the same for Factors 1 and Factor 4. Should these be 8 different projects or can the 5 projects listed for Factor 1 be duplicated in Factor 4?
ANSWER: How ever the offeror determines what would best meet the requirements of the RFQ. If additional projects cannot be provided then duplicate.
Question 9: SF 330: In addition to the listed requirements in the RFQ, a SF330 form was provided as an appendix. Is the SF330 required for the entire team or just the design team? Will the SF330 be counted towards the page count? The SF330 seems to duplicate some of the information already required including resumes (which is already requested on page 10), projects (which is requested under pages 10 – 11 for the design team), and the organizational chart (which is requested on page 10). Do we need to include these again in the SF330 or should duplicated items be omitted from the SF 330 or should there be verbiage noting where to find the duplicated items (i.e. please see resumes under tab 9)?
ANWER: The SF330 is titles Architect and Engineer Qualification, submit this form for those respective disciplines. The SF330 is counted towards the page count. Complete the SF330's in a complete fashion. Complete the other portions of the RFQ in complete fashion.
Question 10: Questionnaires: Can you please clarify the correct amount of questionnaires that needs to go out? Are 4 questionnaires required for Factor 1 page 6 for the GC/Design-Builder or does a questionnaire have to go to both references for a total of 8 questionnaires or just 1 out of the 2 references listed for each of the 4 projects? As for pages 8 – 9 for the lead designer, should there be a questionnaire for each project submitted under Factor 1? Factor 1 requires no more than 5 projects. Does this mean we can submit anywhere from 1 project to 5 projects? For page 9 Factor 1 for the design team, is GSA requesting 1 questionnaire per team member or 2 questionnaires per team member or a questionnaire for each reference listed (the RFQ currently mentions that a minimum of 2 references is required for each design team member)?
ANSWER: Questionnaires should be provided for each reference provided. A questionnaire shall be completed for each reference provided as indicated in the RFQ. Yes, 1-5 projects can be submitted, what ever the offerors will best suit the submittal for the RFQ. A questionnaire must be completed by each reference provided.
Question 11: Past Performance Questionnaires: For the questionnaires, there is mention of the “offeror being evaluated”, for the questionnaires pertaining to the lead designer and the design team key personnel, under “name of offeror being evaluated” is this where we list the offeror for the proposal or the person being evaluated since this is two different firms? Can you please clarify? Does Section 2 on the cover sheet for the questionnaires pertain to the references the questionnaires will be sent to? Can the questionnaires only be returned via e-mail and mail or can these forms be faxed as well? There was no fax number available on the questionnaire.
ANSWER: Section 1 should have the name and company of the person being evaluated. Yes, Section 2 on the cover sheet for the questionnaires pertain to the references the questionnaires will be sent. The questionnaires can be sent vial e-mail, mail or facsimile. The number is 303-236-5328. Please ensure that the cover sheet indicates that it should be delivered to Len Wilson.
Question 12: Past Performance Questionnaires Cover Sheet: The bottom portion that mentions name, company, role, and telephone no., does this pertain to the offeror of the proposal or does this pertain to the reference or the person/firm being evaluated?
ANSWER: The cover sheet is for the offeror of the proposal.
