About this file

The document is a Florida Trench Safety Act Certification and Disclosure Statement, which is a required form for contractors performing excavation work in Florida. The form acknowledges the federal excavation safety standards (29 C.F.R. Section 1926.650 Subpart P) as the interim state standard, and requires contractors to comply with all applicable excavation and trench safety standards. Contractors must disclose their trench safety measures, including the linear feet of trench excavation and associated costs, though these costs are for informational purposes only and do not guarantee additional compensation.

The form emphasizes that the disclosed costs do not fully represent the contractor's total obligation to comply with trench safety requirements. The county's acceptance of the bid does not validate the adequacy of the disclosed costs or relieve the contractor of sole responsibility for meeting safety standards. Contractors are required to provide additional funds at no extra cost to the county if necessary to ensure full compliance with the Florida Trench Safety Act. The form must be completed in full, with failure to do so potentially resulting in the bid being declared non-responsive.

Bidder may use additional sheets as necessary to extend this form. Failure to complete the above may result in the bid being declared non-responsive.

A. The amount disclosed as the cost of compliance with the applicable trench safety requirements does not constitute the extent of the Contractor’s obligation to comply with said standards. Acceptance of the bid to which this certification and disclosure applies in no way represents that the County or its representatives has evaluated and thereby determined that the above costs are adequate to comply with the applicable trench safety requirements nor does it in any way relieve the Contractor of its sole responsibility to comply with the applicable trench safety requirements.
