SAMPLE QUESTIONNAIRE COVER LETTER

The 538 ACSG of the Air Force Material Command is in the process of selecting a contractor(s) for the Thunderbolt Life-cycle Program Support (TLPS) Contract. The TLPS contract will provide a multiple-award indefinite delivery indefinite quantity (IDIQ) contract vehicle to sustain and modernize all configurations of the A-10 weapon system. The A-10 is expected to remain in the AF inventory until at least 2030. This poses unique challenges in sustaining mission capability for an aging aircraft flying well beyond its original design life. All task orders/delivery orders (TO/DO) under this contract will fall into two primary requirement areas: sustainment and development. The TLPS contract vehicle will be used by both the A-10 System Program Office (SPO) and Development System Office (DSO) to address development and sustainment issues for the A-10 weapon system.
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had first hand experience with an offeror, for the evaluation of the offeror's performance on those contracts.
Our areas of interest in the offeror are summarized in the enclosed questionnaire. As discussed in our initial phone contact with your office, our schedule is extremely tight, and we need your written response no later than _______ calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically or by hand and emailed to Capt Ron Hilliard at Ronald.hilliard@hill.af.mil and Andrew Hatch at Andrew.hatch@hill.af.mil. Your completed questionnaire will become a part of the official Source Selection records.
