Dover Air Force Base - Building 200 SOW 21 April 2015

Statement of Work

For the HVAC Renovation of

Bldg. 200 Project Number FJXT111215

At

DOVER AFB, DELAWARE

1.0 SCOPE

The Contractor shall provide all construction services for the renovation of JAG Office Building 200 at Dover Air Force Base, Delaware. The work shall consist of, but not limited to, providing all labor, materials, tools, expertise, testing, surveying, equipment, sampling, warranties, and transportation necessary for the construction of designed renovations to Bldg. 200. The contractor shall construct the renovations detailed in the 100% construction documents provided by Dover AFB as an attachment to this scope. Renovations shall include mechanical, electrical, plumbing, fire suppression, architectural, structural systems upgrades and replacements.

All construction shall be accomplished in a manner that minimizes the impact on the facility users. The facility will remain open during construction. All work shall be performed after normal operating hours (0700 – 1700) and/or on weekends. The construction must accommodate continuity of operations within this facility. All spaces must be clean and ceiling back in place by 0630 during weekdays to accommodate facility operations. All closures and outages must be coordinated a minimum of 30 days in advanced. The Contractor must provide a phasing plan to identify how the impact of construction operations will be minimized for these areas.

1.1 General:

a. The contractor shall complete the renovations identified in the construction documents provided by Dover AFB for the renovation of Building 200.

b. The project shall comply with the Dover AFB Local MILCON and SRM Design Guide dated 1 May 2012, and all other referenced design guides, applicable UFC’s and referenced building codes contained therein.

c. LEED certification will not be required for this project.

d. Temporary Modular Building Swing Space: Temporary office space is not required to be provided by this contract.

1.2 Architectural/Structural:

a. General: The architectural and structural work shall include resealing/weather stripping various doors & windows, sealing various wall penetrations. All flashing, patching, and repair required to accommodate the renovations identified on the attached construction documents shall also be included in this scope.

b. Ceilings: All existing ceilings shall be removed and replaced as part of this renovation.

1.3 Mechanical:

a. Renovate all mechanical work shall include all work identified on the attached construction documents.

b. The contractor will be responsible for providing temporary heating and cooling to all areas occupied during construction (as necessary). All computer and communications room operating temperatures must be maintained throughout construction.

1.4 Electrical Requirements:

a. Renovate all electrical system as identified on the attached construction documents.

1.5 Commissioning Requirements: The contractor shall contract an independent third party commission agent (separate from the TAB contractor) to provide HVAC & plumbing commissioning services on this project. The commissioning agent shall coordinate commissioning activities, review construction documents related to commissioning, including all bid documents. Prepare a commissioning plan and coordination with other subcontractors. Attend commissioning kickoff meetings. Assist in preparing final commissioning plan and issue for use. Help general contractor incorporate commissioning activities into project construction schedule. Work with contractor to minimize user impacts during construction. Attend commissioning progress meetings and review construction submittals and other correspondence including requests for information, change orders, and other relevant project correspondence. Perform a 50% construction site visit. Review TAB, Testing, and Training Plans.

The contractor shall participate in all aspects of onsite commissioning make recommendations for correction, adjustment or modification. Conduct endurance testing of commissioned systems, including review of BAS trends. Schedule and conduct maintenance and operational training sessions. Prepare final commissioning report which includes all test reports, supporting documentation and deficiency resolution.

2.0 APPLICABLE DOCUMENTS

The Contractor shall identify and comply with all applicable Federal, State and local statutes, DoD/Air Force instructions, manuals, handbooks, regulations, guidance, and policy letters including but not limited to:

International Building Code (IBC), Unified Facility Criteria (UFC), National Fire Protection Association (NFPA), National Electrical Code (NEC), and including all changes and amendments in effect on the date of issuance of this contract. It is the Contractor’s responsibility to identify and comply with all applicable requirements.

3.0 GOVERNMENT-FURNISHED INFORMATION, EQUIPMENT, MATERIAL, AND

PROPERTY (GFI, GFE, GFM, GFP)

The Government will provide information in the form of available plans, reports, maps, and drawings as necessary to facilitate accomplishment of the work required in the contract Statement of Work at the Pre- Construction Conference.

The Government will furnish electronic copies of available As-Built drawings that are pertinent to the facility involved. However, these drawings are not warranted to show conditions as they actually exist. The designer shall verify (by field investigation) information shown on the As-Built drawings (which is relevant to the project) for accuracy to ensure a quality-designed project.

The following is a list of Government supplied documents which shall be used for project execution:

a. Most recent version of Dover Air Force Base Local MILCON and SRM Design Guide at time of

RFP.

b. Dover Air Force Base Architectural Compatibility Plan

c. Most recent version of Required Construction Specifications specific to Dover Air Force Base at time of RFP.

4.0 MANAGEMENT, PLANNING, AND REPORTING REQUIREMENTS

The Contractor shall implement a full range of construction and engineering activities, and provide a complete set of construction specification and drawings, in accordance with all applicable compliance documents. The Contractors shall supply all labor, equipment, and materials necessary to accomplish the work assigned unless otherwise specified. The Contractor shall perform management and planning functions, including performance measurement and funds status reporting, through the course of the effort.

4.1 Work Breakdown Structure (WBS)

The Contractor shall prepare and submit, for approval, by the Contracting Office (CO), a WBS consistent with the most recent Construction Specifications Institute (CSI) format for construction activities. The WBS shall be used to report the cost and schedule status for this project. All tasks required under this type of contract shall be included in the WBS. This is the Contract Progress Schedule, AF Form 3064, and Contract Progress Report, AF 3065.

4.2 Schedule and Planning Requirements

The Contractor shall provide schedules for tracking work progress as specified in this Scope of Work.

All features of work on this project shall be completed in accordance with the following schedule.

Construction Activities: 360 days from Notice to Proceed

A final detailed project schedule shall be submitted for approval by the contractor at the start of the project.

4.2.1 Project Planning Chart (PPC)/Construction Planning Chart (CPC).

The Contractor shall prepare and submit a PPC/CPC for approval. The PPC/CPC shall detail the project schedule and status through the use of Gantt charts, which shall depict percentage complete for each task.

The project schedule shall be reported using the approved WBS.

4.2.2 Cost and Status Reporting Requirements for Firm Fixed Price Contract

Cost and status reporting will comply with Dover AFB and AF standards through the submittal of a Contract Progress Schedule, AF Form 3064, prior to the start of construction activities and coordination with BCE to complete biweekly Contract Progress Reports, AF Form 3065, throughout the construction period.

4.3 Meeting and Conference Requirements

The Contractor shall perform a site visit and/or attend a pre-proposal site visit and conference, preconstruction conference, and/or other meetings necessary to support construction activities. The Contractor also shall attend and/or support meeting and/or teleconferences to discuss technical or regulatory issues and project progress and status. The Contractor shall prepare, and submit for review, presentation materials for meetings and an agenda. The Contractor shall prepare minutes for these meetings attended, including teleconferences, and distribute minutes to attendees within 3 working days.

4.4 Contractor Documentation

The Contractor shall create and maintain a Master Document List (MDL) and/or Submittal Register, i.e. AF Form 66, that includes all documents, whether the documents are a deliverable or not, which are prepared during the course of the contract. The MDL and its documents shall be maintained in libraries readily available for submittal to the Government. All Material Submittals shall be accomplished in accordance with the instructions pertaining to AF Form 3000, Material Approval Submittal.

4.5 Notification Requirements

The Contractor is required to notify the CO and Contracting Officer’s Representative (COR) of critical issues that may affect the contract performance an/or human health and the environment. The types of issues that require notification include, but are not limited to, health, risks, spills, changes in critical personnel, and finding unexploded ordnance (UXO). As an example, if UXO were discovered during field activities, the Contractor would be required to immediately stop work, report the discovery to the facility Point of Contact (POC), CO, and COR, and implement the appropriate safety precautions, Field activities could not continue until clearance was received from the CO. On critical issues, verbal notification should be made immediately, followed by written notification as soon as practical.

4.6 Permits

The Contractor shall develop, coordinate, and assist the installation in applying for and obtaining all Federal, State and Local, and other applicable permits, licenses, and certificates required to perform and execute this contract. The Contractor shall maintain a library of these documents as the Contractor’s office as well as the corporate facility. The Contractor shall comply with all applicable permit conditions.

4.7 Photo Documentation

Preconstruction and construction progress photo documentation is required on this project. The contractor shall take detailed preconstruction photos and weekly construction progress photos. All documentation shall be submitted to the government in electronic format on a monthly basis throughout the construction project.

4.8 Site Access Badges

The Contractor shall obtain and monitor assigned security badges (used by both the Contractor and subcontractor staff) used during the duration of this contract. All security badges or passes shall be returned to the base or facility POC upon expiration of the badge, upon completion of the project, or when possession of the badge is no longer necessary (e.g., upon removal of contracted personnel from specific projects).

Special security clearances will not be required for personnel working on the design or construction of this project.

4.9 Worksite Activities and Coordination

4.9.1 Coordination of Activities/Safety Requirements

The Contractor shall coordinate worksite activities with all applicable personnel to ensure the protection of human health and the environment; the prevention of damage to property, utilities, materials, supplies, and equipment; and the avoidance of work interruptions. The Contractor shall provide physical security to work areas with security equipment and personnel.

The Contractor shall be responsible for protecting the lives and health of employees and other persons on the work site; preventing damage to property, materials, supplies, and equipment, avoiding work interruptions and complying with Occupational Safety and Health Administration (OHSA) safety and local safety office requirements. The Contractor shall perform all operation in a prudent conscientious, safe and professional manner and conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work. The Contractor shall maintain documentation supporting training records on site and have written Health and Safety Plans on site and available for workers and/or regulatory review.

The Contractor shall provide the CO copies of any OSHA report(s), regarding a project site, submitted during the duration of the contract. For areas not covered by OSHA, the Contractor shall comply with regulations regarding safety and health and the current edition of the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1.

The Contractor shall record and report promptly (within 1 hour) by telephone, facsimile or other direct means to the CO, COR, the Base POC, and to the military installation’s Safety Office, all available facts relating to each instance of damage to Government property of injury to either contractor or Government personnel. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, preserve evidence until released by the accident/mishap investigation authority through the CO, and immediately report the incident to the Fire Department at 911. If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in conduction of the investigation until it is completed.

The Contractor agrees that his personnel and equipment are subject to safety inspections by Government personnel while on federal property.

4.9.2 Hazardous Material and Hazardous Waste Activities Local Requirements.

The Contractor shall sample and handle the abatement, removal, and disposal of all hazardous materials and waste in accordance with applicable Federal, State, Local and contract requirements. The Contractor shall provide all hazardous materials use and hazardous waste disposal documentation to the installation or facility POC, and shall register with the Hazardous Materials Pharmacy program (if available) at the installation to ensure appropriate and efficient tracking of the Contractor’s hazardous material purchases, inventories, use, and releases such as required by the Emergency Planning and Community Right-to-Know Act (EPCRA), EOs, or any installation reporting requirements. The Contractor shall also comply with Federal, State, and Local requirements for any task involving the transportation of hazardous wastes and/or contaminated materials to off-site treatment, storage, and/or disposal facilities. This includes 40 CFR 260, 49 CFR 172, 173, 178, and 179, and all other applicable Federal, State, and Local transportation regulations.

5.0 PLANS, REPORTS, AND DELIVERABLES

The Contractor shall prepare and submit a Work Plan describing the Contractors plan to implement the work required in this contract. The Contractor shall prepare and submit to the CO for approval, any health & safety, sampling and analysis, waste management plan, construction plan, quality control plan, or other required plans as required prior to receiving a notice to proceed and the commencement of work.

5.1 Health and Safety Plan (HSP)

The Contractor shall provide a Health and Safety Plan (HSP) as specified in this contract.

5.2 Quality System Plan (QSP)

The Contractor shall prepare, for government review and approval, a site-specific QSP. The Contractor must implement, maintain, and comply with the approved site-specific QSP. The Contractor shall organize, document, prepare procedures for, and implement a management-driven system to provide maximum assurance that products and services conform to specified requirements. Products and services submitted in the performance of this contract shall be according to the American National Standard Institute (ANSI)/American Society for Quality Control (ASQC) standards if applicable, or Best Commercial Practice, and contract requirements. All work accomplished by the Contractor shall be of sufficient quality and level of acceptance by Federal, State, and Local regulators or code inspectors. The Contractor shall submit a Quality Control Program Plan detailing the execution of this program to the CO for approval which is required prior to receiving a notice to proceed.

5.3 Technical Plans and Reports

The Contractor shall provide technical plans, specifications, and reports as defined in this contract. Some of these may include, but are not limited to, the following:

5.3.1. Construction Work Plan

The Contractor shall prepare a Construction Work Plan that may include, but is not limited to, any or all of the following subsections as identified in this contract.

5.3.2 Technical Reports

The Contractor shall prepare and submit the following technical reports as identified in this contract.

a. Abatement Plan (If applicable)

b. Hazardous Material and/or Hazardous Waste Disposal Report (Asbestos & Lead)

6.0 SITE WORK

6.1 Conservation

Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, monitoring and other types of wells, and the general site environment.

This includes the protection of trees, shrubs, and other vegetation not in the affected zone from dust damage soil compaction, and physical contact with machines and equipment. If appropriate, the Contractor shall conserve uncontaminated topsoil by removal, storage or redistribution. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work.

All fill materials shall be uncontaminated. The Contractor shall conduct all operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone-depleting chemicals, generation of hazardous waste, and use of energy and water. The Contractor shall plan, construct, operate, maintain, optimize, and decommission systems necessary to control storm water run on and runoff; and transport surface water drainage to a treatment plant, discharge location, or any other destination.

6.2 Site Preparation

The Contractor shall perform site work as necessary to prepare sites for construction activities. Security and access controls shall be implemented to prevent unauthorized entry to sites and to protect wildlife from site exposure. The Contractor shall survey existing utilities to determine adequacy and need for modifications to support site activities. The Contractor shall obtain appropriate approvals and shall construct connections or new systems for electrical power, water, sewer, gas distribution, telephone, and other utilities, as required, to accomplish the activities.

6.3 Demobilization

The Contractor shall demobilize facilities as necessary, and restore the site. The Contractor shall remove any temporary facilities and implement erosion control measures such as seeding, mulching, sodding, and erosion control fabrics; restore roads, structures, and utilities; and plant trees, shrubbery, grasses, and other vegetation. The Contractor shall document and report on activities and train Government personnel to perform required facility equipment operation and maintenance, as requested.

7.0 FACILITY CONSTRUCTION REQUIREMENTS

The Contractor shall perform a full range of activities to meet all construction requirements as designed and specified. Requirements may include design, construction, demolition, repair, and emergency response tasks.

7.1 Build

The Contractor shall manage and perform construction work as defined in the contract. The following are typical construction contract requirements: submittal review, permitting, construction, construction management, scheduling, inspection and testing, maintaining construction facilities and temporary controls, environmental protection, design and construction quality control, and project closeout. Specific requirements will be provided in this contract.

The Contractor will be required to take all meeting minutes and publicize the minutes in accordance with this contract.

The contractor shall function as an integral team member on this project in support of the Dover AFB mission, to include the sharing of information with other governmental agencies and cooperation with communities, and other governmental entities.

The review of construction submittals will be a shared effort between the Designer, the Contractor, and the Government. The Contractor will review all submittals prior to submittal to Dover AFB. Submittal reviews within the Contractor’s team are subject to the Contractor’s determination. The Government will review all submittals including O&M manuals, test reports, etc. The Government will be allowed a minimum of 14 days for the review of all submittals.

Contractor requirements include the efficient management of contracts including accurate, on-time submittals of contract deliverables and timely identification and solution of impediments to successful project execution. Technical requirements include early involvement in the process to allow for the development of the most cost-effective and technically sound solution. The Government will rely on the Contractor’s expertise in recognizing and addressing problematic issues and successful execution of this contract. The Contractor shall perform all work in accordance with Federal, State, and Local statutes and regulations.

8.0 ENVIRONMENTAL

The Contractor shall consult on environmental engineering problems incidental to construction efforts. The Contractor must be able to recognize when specialized environmental expertise is necessary and have the capability of providing that expertise.

9.0 POINTS OF CONTACT (POCs)

Points of Contact will be provided at the kickoff meeting.

10.0 DELIVERABLES

Project deliverables shall be as shown in the table below. A description of each deliverable follows.

10.1 Contract Progress Schedule (AF Form 3064)

The Contractor shall submit a signed AF Form 3064 (or approved equivalent), Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance as described in the Table of Deliverables. The Contractor shall supply one AF Form 3064 prior to start of construction activities on site and will only be required to resubmit due to contract changes. Design will be included only to achieve 100% value on entire project. The Contract Progress Schedule shall be signed by the COR or the Government Contract Inspector recommending approval and approved by the Contracting Officer. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF Form 3065, an appropriate percentage applied, and the item must be received and stored on site.

10.2 Contract Progress Report (AF Form 3065)

The Contractor shall submit a preliminary AF Form 3065 to the BCE inspector as described in the Table of Deliverables prior to each schedule bi-weekly progress review. The BCE inspector will review and supply the Contractor with a Final 3065 based what was provided and reviewed. This report shall be signed by the Contractor’s on-site representative in the Remarks section of the AF Form 3065. The work elements and percentages of the total job identified on the AF Form 3065 shall be identical to the work elements and percentages on the AF Form 3064. The Contractor shall send the signed 3065 to Contracting and BCE.

Each invoice should be based on the most recent approved 3065 and will be entered through Wide Area Work Flow (WAWF). An approved copy of the 3065 and 3064 are not required as part of the WAWF backup. The AF Form 3064, AF Form 3065, and invoice shall all cover the same period of time.

10.3 Kick-Off Meeting/Charrette

The Contractor shall conduct a Kick-Off Meeting/Charrette at Dover, AFB, DE as described in the Table of Deliveries. This meeting will include a complete overview of the project and implications and will be presented to the relevant Base staff and POCs. The Charrette will allow the Government and/or Contractor to discuss any specific concerns not previously addressed. The Charrette shall not be concluded until the Contractor has a complete and workable design concept.

10.4 Construction Progress Meetings

The Contractor shall perform Construction Progress Meetings as described in the Table of Deliverables with meeting minutes provided by Contracting. These meetings will include all relevant Base POCs and

Contracting Personnel on an as needed basis. These meetings will cover the progress made in the past period and will include any pertinent issues which could affect future work progress. The following items shall be covered:

c. RFIs and Statuses

d. Change Orders and Statuses

e. Material Submittals and Statuses w/ Updated Submittal Register

10.5 Format of Deliverables

An electronic copy of all deliverables shall be provided, formatted for use on the latest version and compatible with the Government’s version of Microsoft Word, Excel, Adobe Acrobat, and AutoCad. All deliverables shall also be dated. The Contractor will be provided samples of the deliverables, upon request.
