SECTION SF 30 BLOCK 14 CONTINUATION PAGE

SUMMARY OF CHANGES

SECTION SF 1449 - CONTINUATION SHEET

SOLICITATION/CONTRACT FORM

The required response date/time has changed from 01-Dec-2011 04:00 PM to 31-Jan-2012 04:00 PM.

The following have been modified:

52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)

a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

i) This is a competitive best value acquisition utilizing Performance and Price tradeoff (PPT) procedures. For proposals determined acceptable, a tradeoff between past performance and price will be conducted with past performance being slightly more important than price. This is a competitive best value acquisition utilizing simplified procedures authorized by FAR Part 13 Simplified Acquisitions Procedures. For offers determined acceptable, a best value selection based on price and past performance will be made.

ii) Offeror shall propose on CLINs 0002, 1002, 2002, and 3002 of the SF 1449. In addition, offeror will submit prices for ALL Fixed Priced Items list on Attachment 5. Attachment 2 is used for information only on each line item (description). Any offer that fails to cite a price for any item will be taken as the item is being provided at no charge. Furthermore, the bid can be considered as nonconforming.

iii) Only firm fixed price offers will be evaluated. An offer using a sliding price scale or subject to escalation based on any contingency will not be accepted.

iv) Price: Initially, all acceptable offers will be ranked according to price by adding CLINs 0002, 1002, 2002, and 3002 on SF 1449 and submitted prices listed on Attachment #5. All CLIN extended amounts for the base and all options years will be totaled for evaluation purposes. Price will be evaluated for reasonableness subject to the processes described in paragraphs 4D, 4E, and 4F. Reasonableness will be determined based on prices submitted by the competition, current market conditions, and comparison to the Government estimate, if applicable. Reasonableness will be based on an evaluation of prices to determine if they are compatible with the scope of effort, are unbalanced, and are neither excessive nor insufficient for the effort to be accomplished.

(1) Acceptable : To be determined acceptable for evaluation purposes the offeror must comply with the following:

(a) Ensure CCR & ORCA registration is active & current (not expired) showing NAICS 339999.

(b) Submit One (1) signed copy of the SF 1449 Solicitation (Ensure CLINS show price) and all SF 30 Amendments, if any.

(c) Submit Two (2) copies of the completed COCESS Price List (Attachment 5):

(i) Completed Excel Spreadsheet (Soft Copy-Editable) submitted via E-mail

(ii) Completed Printed & Scanned copy (PDF) via E-mail or FAX

(d) Submit One (1) copy of any details of the Automated Ordering System (AOS)/Online Catalog.

Note: This must include pictures and/or cut-sheets of the proposed AOS/Online Catalog.

b) . Rating Process: The evaluation process will proceed as follows:

i) Past Performance: The contracting officer will seek relevant and recent (within the past 3 years from the issuance of this solicitation) performance information on all acceptable offerors based on references provided by the offeror Attachment 3: “Past Performance Survey” will be used to obtain present and past performance information. Relevancy will be determined based on the criteria listed in paragraph J below, including performance of work efforts involving; COCESS type contract.

Note: The offeror shall send the “Past Performance Survey” to a minimum of one reference.

Note: The offeror not the government is responsible for ensuring their references complete the “Past Performance Survey” and send the survey directly to the 97th Contracting Squadron, Altus AFB, OK before proposal submission deadline. A minimum of one reference sheet per reference (one reference shall be received).

Note: The offeror shall complete one copy each of the attachment four (4) labeled “Reference Sheet” for each reference and submit at least a minimum of one reference sheet overall to the 97th Contracting Office, Altus AFB, OK before the proposal submission deadline.

ii) During the evaluation, process up to three past performance surveys will be evaluated per offeror. The three past performance survey’s will be provided a relevancy rating based on the below relevancy chart.

Figure B.1 Relevancy Chart

Very Relevant
Present/past performance efforts that are similar or greater in scope, magnitude, and complexity than the effort described in this RFP. COCESS contract or similar service at or over $2 million
Relevant
Present/past performance effort involved much of the magnitude of effort and complexities this solicitation requires. COCESS contract or similar service with a value of $500K to $2 million.
Somewhat Relevant
Present/past performance contractual effort involved some of the magnitude of effort and complexities this solicitation requires. COCESS contract up to $500K or similar service
Not Relevant
Present/past performance effort did not involve any of the magnitude of effort and complexities this solicitation requires. NOT A COCESS CONTRACT OR SIMILAR SERVICE

iii) The purpose of the past performance evaluation is to allow the Government to assess the offeror’s ability to perform the effort described in this RFP based on the offeror’s demonstrated present and past performance. The assessment process will result in an overall confidence assessment rating of substantial confidence, satisfactory confidence, limited confidence, no confidence, & unknown confidence (For further information please refer to viii rating process). Past performance regarding predecessor companies, key personnel who have relevant and recent experience, or sub offerors that will perform major or critical aspects of the requirement may not be rated as highly as past performance information for the principal offeror. Offerors with no relevant present or past performance history will receive an “unknown confidence” rating, meaning the rating is treated neither favorably nor unfavorably. However, a rating other than unknown confidence may be assessed if the offeror proposes management personnel who have a record of performance on relevant contracts or if a proposed sub offeror or partner who will be performing, a significant portion of the work has a performance history on relevant contracts. When relevant performance information indicates performance problems, the Government will consider the number and severity of the problems and the appropriateness and effectiveness of any corrective actions taken (not just planned or promised). The Government may review more recent contracts or performance evaluations to ensure corrective actions have been implemented and to evaluate their effectiveness. The offeror may be notified of any adverse past performance information and given the opportunity to respond.

iv) Upon ranking the offers by price if the lowest priced acceptable offer is determined to be reasonable and realistic with a determination to have an overall substantial confidence (For confidence ratings please see figure B.2 Rating Chart) performance rating based on relevancy (see figure B.1 Relevancy Chart) of the past performance, that offer represents the best value for the Government and the evaluation process stops at this point. Award will be made to that offeror without further consideration of any other offers.

v) If the lowest price technically acceptable does not have a substantial rating the following will take place:

(1) The next lowest priced offer and technically acceptable offer will be evaluated and the process will continue until an offeror whose price is reasonable and realistic is rated substantial confidence for past performance. At this time the following would take place:

(2) An integrated assessment will then be performed on all acceptable offerors whose past performance and price were evaluated, and award will be made to the offeror with the proposal determined to be the most advantageous to the Government based on the evaluation criteria.

Note: If no proposals are rated substantial confidence, an integrated assessment will be accomplished on all acceptable proposals and award will be made to the offeror determined to be the most advantageous to the Government based on the evaluation criteria.

Figure B.2 Rating Chart

RATING

DEFINITION

Substantial Confidence
Based on the offeror’s performance record, the government has a high expectation that the offeror will successfully perform the required effort.
Satisfactory Confidence
Based on the offeror’s performance record, the government has an expectation that the offeror will successfully perform the required effort.
Limited Confidence
Based on the offeror’s performance record, the government has a low expectation that the offeror will successfully perform the required effort.
No Confidence
Based on the offeror’s performance record, the government has no expectation that the offeror will be able to successfully perform the required effort.
Unknown Confidence
No performance record is identifiable or the offeror’s performance record is so limited that no confidence assessment rating can be reasonably assigned.

vi) If a lower priced acceptable offer is determined to have an unreasonable or unrealistic price, discussions may be required.

vii) The contracting officer may determine at any point in the process to open discussions. If the contracting officer determines discussions are necessary, all offerors will be evaluated on past performance and price. The competitive range may then be limited to the number of proposals that will permit an efficient competition among the most highly rated offers. Discussions will be conducted, Final Proposal Revisions requested, and the source selection authority will then make an integrated assessment best value award decision.

viii) The Government reserves the right to award a contract to other than the lowest priced offeror.

ix) Offerors may be asked to clarify certain aspects of their proposal (for example, the relevance of past performance information) or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Clarifications conducted to resolve relevance of the past performance information, adverse past performance information, or minor clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.

x) The Government intends to award a contract without discussions with respective offerors. Therefore, each initial offer must address information in accordance with the paragraphs above and should contain the offerors’s best price and past performance reference list. The Government, however, reserves the right to conduct discussions if deemed in its best interest.

c) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). The Government may or may not exercise the option.

d) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
