MEMORANDUM FOR PROSPECTIVE EVALUATORS

SUBJECT: Past Performance Questionnaire
1. The 628th Contracting Squadron, Joint Base Charleston, SC is in the process of selecting a contractor for facilities maintenance support services to include HVAC Preventive Maintenance (PM)/Repair (residential and industrial type), General Building PM/Repair, Electrical PM/Repair, Plumbing PM/Repair, Generator PM/Repair and Boiler
PM/Repair.
2. One of the considerations in proposal evaluation is the verification of the Offeror’s past and present performance on contracts which reflect the Offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an Offeror, for the evaluation of the Offeror's performance on those contracts.
3. Our areas of interest in the Offeror are summarized in the enclosed questionnaire. Please complete the questionnaire by indicating your rating and providing comments wherever applicable. For any ratings above or below satisfactory, please provide an explanation on how the contractor either exceeded your requirement or how the contractor failed to meet your requirement and attach it to your response. This can be a copy of a performance evaluation, discrepancy report, or other documentation. Once complete, please sign the document in the space provided.
4. Prospective offerors may be contacted regarding problem areas or concerns identified to see what corrective actions have been taken or will be taken. However, the individual's name completing the questionnaire will not be disclosed to the Offeror.
5. Your time and effort in providing this information is greatly appreciated and is critical to our evaluation. Please return the completed questionnaire as soon as possible but NLT COB on 16 November 2018 to Lt. Joseph Alexander at joseph.alexander.13@us.af.mil.

D. PERFORMANCE INFORMATION
The Air Force will use your survey responses to determine a level of confidence, based on the offeror’s past performance record, concerning the Offeror’s ability to successfully perform our requirement in accordance with the contract. Please assign the following performance level codes to each of the questioned areas listed below based on the contractor’s performance at your organization or installation. The confidence codes selected will indicate the degree of doubt concerning performance in each area.

Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the Government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

VERY GOOD - The contractor’s performance meets contractual requirements and exceeds some
(requirements) to the Government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S
SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or was satisfactory.
M
MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
UN
UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
N/A NOT APPLICABLE - Unable to provide a score.
Note: Please provide detailed narrative comments to support your ratings. At a minimum, please discuss each response for which you indicated E (Exceptional), M (Marginal) or UN (Unsatisfactory) in response to the questions below (use additional sheets, if necessary).
