The following have been added by full text:
A. The purpose of this modification is to make changes to the bid schedule, CLIN narratives, replace Attachment 2 and provide a conformed solicitation and PWS (dated 10 Aug 15) to submit proposals.
1. The conformed solicitation (70 pages), as attached and uploaded to Fedbizopps with this amendment on
10 Aug 15, will be used to submit your proposal in lieu of the original one posted to Fedbizopps on 8 Jul 15. Please ensure you acknowledge this amendment and that your proposal is submitted using its attachment which has an offer due date of 17 Aug 15, NLT 03:00 PM CST, (see block 8, page 1 of 70)
2. Revision to Attachment 1: PWS paragraph 1.4.6 Sales Receipts, is changed to read:
1.4.6. Sales Receipts. The Contractor shall provide a summary of sale proceeds by type of commodity, along with original sales receipts for recyclable materials and the proceeds to the Government evaluator no later than the tenth working day of the month following the sale month. The sales receipts shall contain the weight, type, and price of the commodity recycled.
3. Addition to Attachment 1, PWS: Paragraph 1.2.1.1 is incorporated in its entirety as listed below. (Provide proposal for this addition in CLINs X011)
1.2.1.1. Trash Compactor. The contractor shall provide one (1) each 30 cubic yard self-contained power trash compactor to include all preventative maintenance and repair of equipment. The compactor will be placed on the provided slab located behind the Medical Center dining hall. The contractor will be responsible for the emptying of the compactor as well as disposal of the generated waste IAW any and all Federal, State, Local, and Base guidelines.
In the event the compactor becomes non-operational, it is the responsibility of the contractor to provide an alternate method of collection and disposal of the trash. The government-provided slab is 10ft wide by 28ft long and is equipped to supply 480 volt, 3-phase power to the contractor-provided control panel. The government will be responsible for hooking up power to the panel.
4. As a result of the changes to Attachment 1, a revised PWS dated 10 Aug 15, is attached and replaces the PWS dated 29 Jun 15.
5. Attachment 2, U.S. Dept of Labor Wage Determination 2005-2301, Rev 15, dtd 22 Dec 14 is replaced with
Attachment 2, U.S. Dept of Labor Wage Determaintion 2005, 2301, Rev 16, dtd 8 Jul 15 (provided with this amendment 0001)

ADDITIONAL INFORMATION FOR COMMERCIAL SOLICITATIONS AND CONTRACTS
PERFORMANCE CONFERENCE
Offerors are hereby advised if they are awarded a contract as a result of this solicitation, they may be required to appear at the 81 st Contracting Squadron, 310 M Street, Bldg 4605, Keesler AFB, MS, to attend a Preperformance
Conference prior to commencement of any work on the military installation.
ADDENDUM TO FAR 52.212-1 INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS
The following paragraphs of this provision are tailored as follows:
(b) Submission of Offers: Offerors will submit one original, hard copy proposal to include all items requested in this paragraph, and as stated in the solicitation. Proposals must be hand-carried or mailed to the address listed in solicitation block 9, page 1 of 70. Email or fax proposals WILL NOT be accepted. Proposals must be signed/dated in blocks 30a-30c of the solicitation with the company’s name/address/phone number and cage code provided in block 17a. All pages requiring fill-in information must be returned with the offeror’s proposal and must be received by the date/time provided in solicitation block 8, page 1 of 70, unless otherwise changed via solicitation amendment. Solicitation amendments must be acknowledged in writing or the proposal may be considered non-responsive. It is the offeror’s responsibility to check www.fbo.gov for amendments issued against this solicitation and respond accordingly.
(c) Period of Acceptance of Offers. The offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(e) Deleted.
ADDENDUM TO 52.212-4 CONTRACT TERMS AND CONDITIONS—COMMERCIAL ITEMS
The following paragraphs of this clause are tailored as follows:
(a) Inspection/Acceptance. Inspection and acceptance of the services will be performed at Keesler Air Force Base, MS, by 81 ID.
Reference clause 52.228-5, INSURANCE - WORK ON A GOVERNMENT INSTALLATION
The Contractor shall, at its own expense, procure and thereafter maintain the following kinds of insurance with respect to performance under the contract.
(a) Workers’ compensation and employer’s liability. Contractors are required to comply with applicable
Federal and State workers’ compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer’s liability section of the insurance policy, except when contract operations are so commingled with a contractor’s commercial operations that it would not be practical to require this coverage. Employer’s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers’ compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.)
(b) General liability.
(1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.
(2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.
(c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.
(d) Aircraft public and passenger liability. When aircraft are used in connection with performing the contract, the contracting officer shall require aircraft public and passenger liability insurance. Coverage shall be at least $200,000 per person and $500,000 per occurrence for bodily injury, other than passenger liability, and
$200,000 per occurrence for property damage. Coverage for passenger liability bodily injury shall be at least
$200,000 multiplied by the number of seats or passengers, whichever is greater.

(e) Vessel liability. When contract performance involves use of vessels, the contracting officer shall require, as determined by the agency, vessel collision liability and protection and indemnity liability insurance
Reference Clause 52.237-1, SITE VISIT
A site visit will be held Thursday, 23 Jul 15, 9:00 AM CST. All interested offerors are encouraged to inspect the site prior to submitting a proposal. If you plan on attending the site visit, please email both Kimberley Alvarez at kimberley.alvarez.1@us.af.mil and DaVina Davis at davina.davis@us.af.mil no later than 1200 PM CST on 21 Jul
15 to confirm attendance. Further reporting instructions will be provided upon confirmation. Each vendor is limited to two attendees. The number of attendees, to include name/phone/company name, should be included in the confirmation. Every effort will be made to arrange government transportation to/from the site visit; however, if transportation cannot be arranged or is cancelled in the event of mission requirements, please be prepared to obtain a base pass by bringing required documentation necessary to gain access. Required documentation is all of the following: a valid driver’s license, current vehicle registration, proof of insurance and social security card must be provided for each vehicle. All individuals entering the installation must have picture identification. Technical and
Contracting personnel will attend the site visit and all questions/answers received will be posted to www.fbo.gov.
DEADLINE FOR SUBMISSION OF QUESTIONS

This attachment is provided for interested offerors to submit past performance contact information in a uniform format. The information provided by the offeror may be used to obtain additional past performance when no CPAR record is available. Offerors are encouraged to submit past performance references (no more than 3) in Attachment
3 and return with their offer.
