The following have been modified:

INSTRUCTIONS TO OFFERORS

L-900. INSTRUCTIONS TO OFFERORS

I. Proposal Format

A. General: The following instructions cover the preparation and submittal of the offeror’s proposal for this solicitation. The term “offeror” means the prime contractor submitting the proposal. Offerors must follow the instructions contained herein. Offerors are cautioned that any noncompliance with the terms and conditions of the

Request For Proposal (RFP) may cause their proposal to be determined to be unacceptable and therefore not considered eligible for award. If a joint venture arrangement exists for this acquisition, the Offeror shall provide a copy of the joint venture agreement that is signed and dated by all joint venture members as part of the proposal submission. Offerors shall submit only one proposal for the “Repair Building Envelope” as the Government will review only one proposal per offeror. Submit two (2) original hard-copy proposals, and two (2) electronic copies

(CD-ROM/DVD). Proposals must be received no later than the date and time specified in Block 13 on the face page of the RFP. Offerors are requested to submit Volume III, Past Performance Information, so that it is also received by 30 July 2018. No faxes will be accepted.

Mail Proposals to:

CCAFS/Bldg. 1704, MS 2037

Patrick AFB, FL. 32925-2206

Attention: Rhonda Davis and/or Patricia Bates

PROPOSALS ARE DUE BY 12:00, 30 JULY 2018.

B. Communications:

1. Exchanges of source selection information between Government and Offerors will be controlled by the

Contracting Officer. Email may be used to transmit such information only if the email can be sent encrypted, and must include “Source Selection Information – See FAR 2.101 and 3.104” in the subject line of the email.

Otherwise, source selection information will be transmitted via direct mailing. In order to facilitate the sending and receiving of encrypted emails, Offerors must use MS Outlook email configured to support encryption or a different email product that is S/MIME compatible and configured to support encryption. If you intend to submit your source selection information via encrypted email, you will need to contact the Buyer/Contracting Officer indicated on the face page of the solicitation prior to that first submittal in order to exchange certificates used for encryption. To ensure the process is working correctly, send a test encrypted message first (without including any source selection information). Otherwise, source selection information will be transmitted via direct mailing or

AMRDEC SAFE at https://safe.amrdec.army.mil/safe/guide.aspx. Any communication between the Offeror and the

Government, such as an Evaluation Notice (EN), must be transmitted in accordance with these directions.

2. To enable the Government to successfully view the proposals electronically, the Offeror shall submit their proposal in Adobe Portable Document File (PDF) or Microsoft Word AND Microsoft Excel format for Volume I –

Completed RFP, Volume II – Technical Proposal, and Volume III – Past Performance. All three volumes must be on each CD-ROM/DVD for a total of two (2) copies. The Government will use the electronic copies and paper hard-copy in the evaluation process. However there is no requirement for the Government to compare or verify the consistency of the electronic proposal to the paper proposal. In the case of an identified dispute or variation in content, the paper/hard-copy proposal submitted shall be deemed and treated as the official version for all purposes.

Label each CD Case and the CD itself with Offeror’s Company name and numbered sequentially in the required number of copies.

3. The Offeror shall set all security options in each PDF file to “allowed”. All text, including table and figure identifiers, shall be indexed and 100% searchable text. ALL text shall be displayed as a font and not displayed as a bitmap in any proposal volume or appendix to facilitate the ability to copy and paste to another document. The Price

Schedule using Attachment 9, Price Schedule shall be submitted “unlocked” and in the ‘Excel’ format provided in the solicitation. The Offeror shall not embed sound or video files into the proposed files. Minimize the use of scanned images and keep embedded graphics as simple as possible. The Offeror may include a PDF version of the

Price Schedule, Attachment 9.

Completed RFP shall consist of the completed and signed RFP, to include any amendments issued, with a cover letter delineating any exceptions taken to the RFP terms and conditions with accompanying rationale. However, offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined unacceptable and therefore not considered eligible for award.

2. Section B: The Offeror shall prepare a unit cost for each line item on the Price Schedule and enter proposed unit pricing in “Price Schedule”, Attachment 9. Populate the “UNIT COST” column for each of the tabs called “Bid

Schedule – 1Aug18-31Jul19”, “Bid Schedule – 1Aug19-31Jul20”, “Bid Schedule 1Aug20 – 31Jul21”, and “Bid

Schedule 1Aug21-1Feb22”.

The quantities have been provided in the Price Schedule. The Summary Tab will populate automatically. It is the

Offeror’s responsibility to ensure all “TOTALS” are correctly calculated. All unit costs are fully burdened. The term “fully burdened” includes but is not limited to, all labor, materials, equipment, other direct costs, subcontracts, indirect costs, overhead, profit, and bond. 4. Section K: Complete the representations, certifications, and acknowledgements. Section K shall be returned in its entirety. If the representations, certifications, and acknowledgements (Section K) are completed through System for Award Management (SAM.gov), then Section K need not be submitted in its entirety. Instead attach a copy of the SAM.gov representations, certifications, and acknowledgements and any completed copies of the provisions not included in the SAM.gov printout.

6. Section M: Section M, Evaluation Factors for Award spells out in detail how the proposals will be evaluated and

Basis of Award.

7. Provide company/division’s street address, county, facility code; CAGE code; DUNS code; TIN; size of business

(large or small); small business status and labor surplus area designation. This same information must be provided if the work for this contract will be performed at any other locations(s). List all locations where work is to be performed and indicate whether such facility is a division, affiliate, or subcontractor, and the percentage of work to be performed at each location.

8. The Offeror will provide a description of work they intend to perform within their own organization to meet the requirements of FAR clause 52.219-14, Limitations on Subcontracting.

9. Provide the name, title, and telephone number of the company/division point of contact regarding decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the Government.

C. Volume II, Written Technical Proposal

1. General: The written technical proposal shall be clear, concise, and include all the information required by this provision in sufficient detail for effective evaluation. The proposal should not simply rephrase or restate the

Government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume the Government has no prior knowledge of their capabilities, work processes, facilities, and experience and will base its evaluation on the information presented in the offeror’s technical proposal.

The Source Selection Technical Team shall evaluate the technical proposals on an acceptable or unacceptable basis assigning one of the ratings described below for each sub-factor. Any sub-factor evaluated as “Unacceptable” will render the entire proposal unacceptable and, therefore, not awardable. Only those proposals determined to be technically acceptable, either initially or as a result of discussions, will be considered for award. However, the offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion.

2. Format and Specific Content: The following information shall be provided in the written technical proposal and will be evaluated to assess technical acceptability in accordance with Section M-900, Evaluation Basis for

Award. The written technical proposal shall address each of the following sub-factors to describe the offeror’s proposed approach to performing the requirements set forth in Section M, Evaluation Factors for Award.

Element 1 – Sample Project:

Description: The Sample Project documents are Attachments 10, Sample Project SOW & Response Form, and

Attachment 11 Sample Project Drawings. This sample project is similar in size, scope, and complexity to the work described in the Specifications of this Solicitation. This sub-factor evaluates the offeror’s technical competency to demonstrate knowledge and understanding of work tasks and prepare a Schedule of Work that shows the appropriate sequence of tasks required to complete the sample project. This Sub-factor also evaluates the offeror’s technical ability to identify work tasks and choose appropriate line items from the price Schedule (Attachment 9).

Measure of Merit: The first requirement for acceptability, Offeror shall demonstrate the ability to effectively identify, plan, and schedule the various aspects of the project. Prepare a proposed schedule of work for the tasks required to complete the Sample Project SOW (Attachment 10). Each unique work task shall be reflected in the appropriate sequence. The tasks shall be presented in the order they would need to occur for satisfactory completion of the Sample Project, and include a Start Day, End Day, duration for each task, and a narrative describing any assumptions that were made concerning the task or the project. The Offeror must identify the Majority of tasks in the correct sequence and not exceed the stated period of performance to be technically acceptable. Lines may be added to the Response From as needed.

The second requirement for acceptability, Offeror shall identify the appropriate Item Numbers from the Price

Schedule (Attachment 9) to accomplish the Sample SOW requirements. The Majority of Item Numbers must be appropriately identified for the proposal to be technically acceptable.

An offeror’s proposal that fails to demonstrate the ability to effectively identify, plan, and schedule the various tasks of the Sample Project, AND demonstrate technical competency to identify the Majority of the appropriate line item that corresponds to the work tasks for the Sample Project, will be considered unacceptable.

Element 2: Management Capabilities

Description: This sub-factor evaluates the offeror’s key personnel qualifications to perform work similar in size, scope, and complexity to the work described in the Specifications of this Solicitation (Management Plan). This sub-factor also evaluates the Offeror’s Quality Control Program, Fall Protection and Prevention (FP&P) Plan, Roof

System Applicator Certifications, and Specialized Equipment and Processes to perform work similar in size, scope, and complexity to the work described in the Specifications of this Solicitation. All of these Management

Capabilities must be identified for the proposal to be technically acceptable.

Measure of Merit:

a. Key Personnel:

i. The first requirement for acceptability is offeror identification of key personnel and the required qualifications, licenses, certifications, and experience to qualify these individuals as key personnel with experience performing work similar in size, scope, and complexity to the work in the Specifications of this Solicitation.

ii. At a minimum the Offeror shall identify as specified in the solicitation, the Superintendent, Quality

Control Manager, and Site Safety and Health Officer (SSHO).

iii. Key personnel shall have a minimum of three (3) years of experience in the necessary skills and shall be familiar with the requirements and methods for proper performance of the work in the IDIQ Specifications. The

SSHO shall meet the requirements of Specification Section 01 35 26, Paragraph 1.6.1.1. Demonstrate compliance with this requirement by submitting evidence the key personnel have three (3) years’ experience in their key area.

iv. If the same individual is proposed to fulfill more than one “key personnel” position, submit evidence of sufficient experience in all positions they are fulfilling.

b. Quality Control Plan:

i. Offeror shall describe the company quality control program and how it will be implemented to ensure successful performance under this IDIQ.

ii. Describe how non-conforming work will be identified, documented and corrected.

c. Fall Protection and Prevention Plan:

Provide a typical FP&P plan for a recent project completed by Contractor or Subcontractor or provide a

FP&P for the Sample Project Drawings (Attachment 11) per Specification Section 01 35 26, Paragraph 1.7.1.e.

d. Roof System Applicator Certification:

Provide roof system applicator certifications that show the contractor or subcontractor is an approved applicator for the following three (3) roof types specified in specification sections: 07 41 13, Metal Roof Panels; 07

52 16.14, SBS Modified Bituminous Membrane Roofing Hot – Mopped Asphalt; and 07 54 23, Thermoplastic

Membrane Roofing or certifications of potential “approved equal” for these three roof systems.

e. Specialized Equipment and Processes:

Describe the company’s capability to address large complex industrial facilities and specialized tasks.

Specifically, Offeror is to describe how requirements for crane and “high reach” equipment for projects over 50 feet high will be met.

An Offeror’s proposal that fails to identify acceptable Key Personnel, Quality Control Program, Fall Protection and

Prevention Plan, Roof System Applicator Certifications, and Specialized Equipment and Processes demonstrating an understanding of the size, scope, and complexity of the work described in the Specifications of this Solicitation will be considered unacceptable.

D. Volume III - Past Performance

1. General: The offeror shall submit Present and Past Performance Information for itself and any joint venture member, as well as each proposed critical subcontractor in accordance with the format contained in the Past performance Questionnaire (PPQ) (Attachment 7) and the following paragraphs. A critical subcontractor is defined as an entity (subcontractor and/or teaming contractor), other than the offeror itself that will perform painting and/or roofing.

2. Past Performance Format: The requested present and past performance information shall be provided in a separate volume labeled "Volume III - Past Performance". A summary page shall be provided for this acquisition, describing the proposed role of the offeror, any joint venture member and critical subcontractor

(nature of work and percentage of overall work). Summary page information must clearly communicate that proposed critical subcontractor(s) meet the definition of a critical subcontractor established above. Efforts submitted for proposed critical subcontractors not meeting the established definition will not be evaluated. The first page of

Volume III – Past Performance shall include the company name and point of contact phone number and/or email for each Past Performance Questionnaire sent out. Past Performance Questionnaire responses from

POCs/customers should be returned in accordance with paragraph four (4) of this section.

Each offeror/joint venture member shall complete a separate PPQ for no less than three (3), no more than five (5) active or completed contracts (with at least one year of performance history) in the past three (3) years from the issuance date of the RFP, that the offeror/joint venture member considers relevant in demonstrating its ability to perform the proposed effort. For each critical subcontractor included in the offeror's proposal, a PPQ for no less than three (3), no more than five (5) of their most recent and relevant contracts shall also be included. The offeror may not submit more than ten (10) total PPQ’s for both Prime and Sub-Contractors. The offeror's/joint venture member’s or critical subcontractor’s present and past performance information may include data on efforts performed by other predecessor companies, affiliates, other divisions or corporate management if such was provided for evaluation and if the offeror’s past performance volume demonstrates the company, affiliate, or division will provide the offeror with resources for the instant proposed effort, such as workforce, management, facilities, or other capabilities demonstrating direct and meaningful involvement in the performance of the instant proposed effort.

The PPQ shall clearly indicate the division or corporate organization that performed or is presently performing the contract. Contracts listed may include those with the Federal Government, state and local governments or other agencies, and commercial customers. It is important that the offeror’s past performance information be accurate, complete, and in the correct format. If the information is not submitted in the correct format, the Government will not be able to evaluate the effort for recency and relevancy. There are specific places for submission of this information in the PPQ (Attachment 7). This includes but is not limited to information such as the dollar value, period of performance, type of contract, and program title and description.

3. Subcontractor/Teaming Member Consent Form: In addition to the information provided in the PPQ

(Attachment 7) for each entity as required above, the offeror must submit a consent letter executed by each of its proposed teaming member(s) and/or critical subcontractors authorizing release of adverse past performance information to the prime offeror to allow the prime offeror an opportunity to respond. A sample

Subcontractor/Teaming Partner Consent Form is attached to this RFP (Attachment 8). The consent form shall be completed by the team member(s) and/or critical subcontractors identified in your proposal. The completed consent forms shall be submitted as part of your Past Performance Volume III.

4. Past Performance Questionnaire (PPQ): The Present/Past Performance Questionnaire (Attachment 7) will be one means used by the Government to obtain present/past performance information. The Government reserves the right to change, alter, and/or supplement the questionnaire without further notice to the offeror(s). The offeror shall send out and track the completion of the Present/Past Performance Questionnaires (Attachment 7) to each of the offeror’s, joint venture members and/or critical subcontractors’ (i.e., each entity’s) Points of Contact (POCs) identified in each PPQ. The responsibility to send out and track the completion of the Present/Past Performance

Questionnaires rests solely with the offeror - i.e., it shall not be delegated to any other entity. The Transmittal

Letter shall be used by the offeror in sending out the Present/Past Performance Questionnaires (Attachment 8).

Exert your best effort to ensure that at least two POCs per relevant contract submit a completed Present/Past

Performance Questionnaire directly to the Government not later than the date established in the RFP for receipt of proposals. POCs may submit their completed Present/Past Performance Questionnaire either electronically or by mail. If sending electronically, follow the procedures outlined in paragraph I.B.

Communications above, and email to: rhonda.davis.4@us.af.mil, kristin.doty@us.af.mil, and mailto:rhonda.davis.4@us.af.mil mailto:kristin.doty@us.af.mil patricia.bates@us.af.mil. Electronic submissions are preferred, but if mailing, the POCs may return the completed questionnaires via mail directly to:

5. Past Performance Information: The offeror/joint venture member or critical subcontractor, if applicable, shall focus its PPQ responses so that they clearly correlate present and past performance with the requirements of this

RFP. The PPQ responses must clearly describe the relevance of the effort to the work proposed. The “Remarks”

Section on the PPQ may be filled-in for each relevant contract that covers no more than both sides of three (3) 8 ½ x

11 inch pages (i.e., no more than six (6) pages). Provide the most current information for the Points of Contact

(POCs) identified on the PPQ. At least two of the following (in descending order of availability) should be identified as current POCs on the PPQ:

(a) Procuring Contracting Officer/Contract Negotiator or equivalent

(b) Program/Project Manager, or equivalent

(c) Administrative Contracting Officer/Contract Specialist or equivalent

If problems were encountered during the performance of the identified contracts, provide evidence of the ability to isolate the root causes of problems and include in the PPQ a description of programs or actions taken to resolve those causes. Problems not addressed in the PPQ, but found by the Government during the evaluation of the information in this volume, will be assumed to still exist. Note: In the case of Contractor Performance Assessment

Report System (CPARS), if your input has already been provided and the rationale/circumstances have not changed, DO NOT repeat them here.

Once the Present/Past Performance Questionnaires are completed by your POCs, the information contained therein shall be considered source selection sensitive and shall not be released to you, the offeror. Therefore, any exchange/contact between the offeror/joint venture member and/or critical subcontractor and its own

POCs in regards to comments made on the questionnaire is not permitted.

6. Follow-up Discussions: The Government may conduct follow-up discussions with any of the people identified in the PPQ or in the offeror’s Present/Past Performance Volume. Pursuant to FAR 15.305(a)(2)(ii), the Government may obtain other information by sending out additional questionnaires and through other sources, other than those listed within the PPQ.

7. RFP Attachment Legend: Attachment 7 Past Performance Questionnaire must include the following legend at the top and bottom of the page:
