About this file

This document contains an experience questionnaire template and solicitation for the HAMP CUPOLA WOODWORK REPAIR project. The experience questionnaire is a template for offerors to list up to three similar past projects over the last five years, including project descriptions, performance remarks, and up to six photographs. The solicitation seeks proposals from construction firms to repair wood elements of the mansion cupola at Hampton National Historic Site in Towson, Maryland. The work must be completed in accordance with attached specifications and adhere to historic preservation standards. Firms must have experience working on historic structures. The contract is for a firm-fixed price between $1-5 million and requires all necessary tools, labor, equipment, insurance, bonds, and materials. The site visit will be on August 4, 2020 and questions are due by August 7, 2020. Proposals will be evaluated on technical merit and price, with award going to the best value. The project is set aside for certified small disadvantaged businesses.

Text version

Experience Questionnaire READ CAREFULLY: The purpose of this form is for the offeror to list projects performed during the past five (5) years that are similar to the work described in this solicitation. Provide a minimum of three projects. Along with the project description submit no more than 6 photographs illustrating the work. At least one photo must show the before conditions of the project to illustrate the extent of the work. Use this form to address your familiarity with the Project Specifications and with the evaluation criteria described in the solicitation. Include Area Codes with all telephone numbers. Fill in the information by typing or printing legibly.

It is especially important that the offeror disclose instances in which their experience may be considered by others to be less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror’s experience.
