About this file

The documents provided are Exhibits C through F for a uniform and mat rental and laundry services contract solicitation by the San Diego County Water Authority. The exhibits include a detailed Cost Proposal Form (Exhibit C), a form for Exceptions/Alternatives to Scope of Work (Exhibit D), a Subcontractor Designation Schedule (Exhibit E), and a Proposer Questionnaire (Exhibit F). The contract covers weekly rental services for various items including 100% cotton work clothing, flame-resistant coveralls, shop towels, and multiple sizes and types of floor mats for two Water Authority locations.

The Cost Proposal Form (Exhibit C) outlines specific pricing details, including weekly rental rates for clothing and mats, additional charges for emblem preparation, name badges, and tailoring, and an optional purchase item for a soiled laundry bin. The document includes provisions for a potential fuel surcharge when diesel prices exceed $4.25 per gallon and allows for a contract extension with price adjustments based on the Consumer Price Index, limited to 3% or the actual CPI change, whichever is less. The Proposer Questionnaire requires bidders to provide details about their service capacity, including plant locations, number of employees, current operating capacity, route drivers, and ability to service the specified locations with brown nylon mats.

Cost Proposal Form Notes:

a. All transportation, fuel, and delivery costs shall be included in the unit weekly rental prices above.

Mesa locations weekly invoices when the most recent weekly average for California diesel fuel (on highway) ultra-low sulfur price exceeds $4.25 per gallon as published by the U.S.

Energy Information Administration at http://www.eia.gov/petroleum/gasdiesel/.

Contractor is solely responsible for adding the fuel surcharge. Fuel surcharges may not be added retroactively to paid or delivered invoices. Water Authority may deduct fuel surcharges for weeks when the most recent weekly average has dropped below $4.26 per gallon.

c. Items 11-15 shall be free for employees in the following instances: 1) at contract startup,

2) for new employees, and 3) for normal wear and tear (not caused by employee) replacement. Items 11-15 shall be chargeable for damage replacement or lost items caused by Water Authority, or size changes for employees who are not new.

d. For Items 12 and 13, printed name and company logo badges are preferred. If proposer only provides embroidered name and company logo badges, please clearly indicate on Exhibit D.

e. Special size, stockroom, or any other fees or charges will not be allowed.

f. Contractor is solely responsible for providing timely, accurate invoices per the contract terms. Invoices with inaccurate pricing or that are otherwise not per contract terms shall not be paid until revised to contract terms and pricing.

g. If the option to extend the contract term for thirty-six (36) additional months is exercised, the cost per item for all Cost Proposal Form Items will be adjusted, based on the Consumer Price Index for the San Diego area, all urban consumers, base: 1982-84=100, not seasonally adjusted (CPI-U), or 3 percent, whichever is less. The adjustment will be based on the most recent published over-the-year percent change (published annually in February). If the over-the-year percent change is less than zero, no adjustment shall be made to the Contractor’s rates. Rate increases shall not change the maximum contract amount.
