About this file
This file is an evaluation form and satisfaction program document created by the City of Toledo to assess the performance of contractors and consultants providing services to the municipality. The Contractor/Consultant Satisfaction Program is designed to evaluate performance, increase communication, foster positive relationships, and communicate expectations regarding timely, cost-effective, and quality service delivery. The evaluation instrument is included in all bid packets and requests for proposals and is discussed at pre-bid and pre-construction meetings. Within 30 days of project completion, the employee responsible for contract oversight completes the evaluation form and, if necessary, conducts a post-contract meeting with the contractor or consultant to review results. Evaluation files are maintained by individual divisions and entered into a database on the City's public file server for access by all divisions and potential use in future bid award processes.
The evaluation form assesses performance across four primary categories: Organization and Management (12 criteria with a maximum of 140 points), Equipment (2 criteria with a maximum of 20 points), Work Performance (9 criteria with a maximum of 160 points), and Subcontractor Management (5 criteria with a maximum of 50 points), for a total possible score of 370 points. Evaluation criteria include management availability and competence, staffing adequacy, timely documentation, respect for City authority, compliance with wage and labor regulations, work site safety and cleanliness, traffic control maintenance, contract compliance, quality of work, schedule adherence, budget performance, equipment appropriateness and reliability, subcontractor coordination, and Disadvantaged Business Enterprise utilization. Each criterion is rated on a scale of 1 to 10, with weighting factors applied to certain items such as schedule communication (factor of 2), City direction compliance (factor of 2), schedule respect (factor of 3), budget performance (factor of 3), and quality of work (factor of 2).
