EXHIBIT C – WRITTEN PROPOSAL

Maximum Points for Qualifications: 730

1. Technical Proposal Specifications (Maximum available points: 400)

The bidder must provide a prior work product example for the following three types of assessments (1-3) and use each subcategory (A-C) once.

Assessments (1-3) must use each subcategory (A-C) once. Each work product example must be clearly identified using the Assessment number; a dash; and the subcategory used in that example (e.g. 1-B; 2-C; or 3-C)

1.1 Medical or surgical device or medical equipment Subcategories:

A. Must contain one example related to an emerging intervention; OR B. Related to a currently used or more commonly accepted intervention that is outdated or being used in new ways; OR C. Includes a cost effectiveness analysis component. (Maximum available points: 50)

1.2 Medical or surgical procedure Subcategories:

A. Must contain one example related to an emerging intervention; OR B. Related to a currently used or more commonly accepted intervention that is outdated or being used in new ways; OR C. Includes a cost effectiveness analysis component. (Maximum available points: 50)

1.3 Diagnostic test Subcategories:

A. Must contain one example related to an emerging intervention; OR B. Related to a currently used or more commonly accepted intervention that is outdated or being used in new ways; OR C. Includes a cost effectiveness analysis component. (Maximum available points: 50)

Bidder must provide templates of a proposed document to be used for this contract for the items listed below. Include brief descriptions of major topic areas that would be included in the proposed template:

1.4 Health technology assessment; (Maximum available points: 50)

1.5 Oral/slide presentation of assessment for a clinical committee audience; (Maximum available points: 50)

1.6 Work plan for each assignment; (Maximum available points: 50)

1.7 Periodic status reports on progress against the work plan; and (Maximum available points:

50)

1.8 Additional sample of related HTA work. (Maximum available points: 50)

2. Management Proposal Specifications (Maximum available points: 330)

2.1 Describe the position within the entity that will have prime responsibility and final authority for the work, and the next senior level of management. (Maximum available points: 30)

2.2 Provide a copy of the Proposer’s Conflict of Interest policy, guideline, or agreement that Proposer requires of its team members. (Maximum available points: 30)

2.3 Provide a description of the project team structure and internal controls to be used during the course of the project, including an indication of potential subcontractors. Indicate position types (including identification of the labor categories below), responsibilities, time commitment estimated, and minimal qualifications for typical assessment projects.

2.4 Describe staff, including subcontractors who may be assigned to conduct the HTAs and related tasks that have general and specialized clinical, behavioral, social sciences, economic, and management expertise. (Maximum available points: 30)

2.5 The bidder must identify an executive with qualifications of a Labor Category 1 that possesses strong corporate level management experience. (Maximum available points: 30)

2.6 The bidder must identify a project manager or managers, if different from the executive that will be responsible for the day-to-day management of individual assignments. (Maximum available points: 30)

Resumes must include information on the individual's particular skills related to this project;

education; experience; significant accomplishments; and any other pertinent information must be submitted.

2.7 Provide resumes for executive and managers of day-to-day management of individual assignments. (Maximum available points: 30)

2.8 Representative samples of two resumes for each Labor Category (LC 1-4) of staff that are currently committed to the entity and that may conduct the health technology reviews.

2.9 Provide a list of contracts the Bidder has had during the last three years for health technology or evidence-based reviews. List brief description of work, contract period of performance, contact person, telephone number and email address. (Maximum available points: 30)

2.10 Provide a summary of relevant experience that demonstrates the qualifications of the bidder and any subcontractors to conduct scientific evidence-based health technology assessments and updates; provide consultation and input in formulating key questions and other preliminary matters; and report, including oral presentations, on the findings of the health technology assessments. (Maximum available points: 30)

2.11 Demonstrate at least three years’ performance of evidence-based reviews, preferably focused on health technologies. Experience in performing or incorporating the results of cost-effectiveness analysis is required. Specific experience in producing health technology assessments to assist health care payer organizations is desired. Experience in analyzing any unique impact of an intervention on specific populations based upon factors such as gender, age, ethnicity, rate, or disability is desired. (Maximum available points: 30)
