About this file

This document is an Itemized Service Budget (ISB) instruction template, specifically designed for a contract with the Arizona Department of Economic Security (DAAS/ADES). The document provides detailed guidance for contractors to complete a comprehensive budget breakdown for service delivery, outlining specific categories and requirements for cost allocation and reporting.

The ISB template allows contractors to document service costs across nine primary budget categories: Personnel, Employee Related Expenses, Professional Services, Travel, Space, Equipment, Materials and Supplies, Operating Services, and Indirect Costs. The document emphasizes precise cost tracking, requiring contractors to differentiate between administrative and direct service functions, allocate full-time equivalent (FTE) percentages, and identify all revenue sources. Contractors can use either a federally negotiated indirect cost rate or a 10% de minimis rate, with specific documentation requirements. The budget form enables granular expense tracking, including specific allowable costs within each category, and provides a structured method for contractors to detail their anticipated expenses and funding sources for service delivery.

If the subrecipient elects to use a federally negotiated indirect cost rate, they must provide a copy of the rate agreement with the ISB. If the federally approved indirect cost rate changes or is allowed to expire within the contract period, the subrecipient must notify DES of the changes. If the subrecipient elects to use the 10% de Minimus rate for indirect costs, they must complete and submit the Attachment 9 Certification of De Minimus Indirect Cost Rate form.
