About this file

This is a Bid Form (Exhibit B-1) for an HVAC Evaluation for Repair and/or Replacement contract opportunity issued by Clifton T. Perkins Hospital Center in Maryland. The solicitation seeks a contractor to evaluate and provide expert services for the repair and/or replacement of multiple malfunctioning air conditioning units throughout the facility. The bid form requires contractors to submit pricing for labor rates for both Master/Licensed Journeyman HVACR technicians and Apprentice HVACR technicians, along with material cost estimates for six specific HVAC units across different facility locations: a Trane unit at the IT Building, a Mitsubishi Electric mini-split heat pump at the Control Center, a Mitsubishi Electric unit at the Greenhouse, and three Reznor and Carrier units at various Plant Management locations. The contract is structured as a time and materials agreement with a maximum total award amount of $50,000.

All bid prices must be fully loaded to include labor, overhead, administrative costs, fuel surcharges, travel expenses, and technician rates, with no additional amounts payable beyond the quoted prices. Material pricing submitted is for estimation and evaluation purposes only and may fluctuate, while labor rates must remain fixed for the contract term. Bidders must complete the form exactly as provided without alterations or blank items, with all unit and extended prices clearly entered in dollars and cents, calculations rounded to the nearest cent, and the form signed and dated by an authorized representative. The bid form explicitly prohibits contingencies or conditions on pricing, and failure to adhere to any instructions may render the bid non-responsive.
