About this file

This is a General Certifications form (Pro Form 026) used by the Los Angeles County Metropolitan Transportation Authority (Metro) for state and local contract opportunities. The document serves as a certification checklist that bidders and proposers must complete in response to Metro's Invitations for Bid (IFB) or Requests for Proposal (RFP). Bidders are required to respond affirmatively or negatively to five certification categories and provide written explanations for any negative responses. The form does not specify particular products, services, quantities, response dates, site visits, or award information, as it functions as a standardized compliance document applicable across multiple Metro contracting opportunities.

The certifications address nondiscrimination in employment practices, compliance with Fair Employment and Housing Act requirements, Iran investment activity prohibitions for contracts valued at $1,000,000 or more, fraudulent use of Disadvantaged Business Enterprise (DBE) fronts, and whistleblower protection provisions under California Labor Code Section 1101. Bidders must execute the form under penalty of perjury, with signature authority required from an authorized official. The document includes contact information for reporting fraud, waste, or abuse to Metro's Small Business Diversity and Labor Compliance Office, the Metro Inspector General Office, and the U.S. Department of Transportation Office of Inspector General. No pricing information, set-asides, funding sources, or contract terms are specified in this certification form, as it serves as a prerequisite compliance document rather than a standalone contracting document.
