About this file
This is an Exhibit G Respondent Reference Form for Solicitation No. HHS0017808, issued by the Texas Department of Family and Protective Services (DFPS) for an Agileyx Renewal contract. The form requires respondents to submit a minimum of three business references demonstrating their ability to perform the scope of work outlined in the Invitation for Bid (IFB). References must cover services provided within the last two years and include complete contact information for customer points of contact, including names, titles, phone numbers, and email addresses. Respondents must clearly indicate whether they performed each referenced work as a prime contractor or subcontractor, and if applicable, provide a detailed description of the scope of subcontracted activities for each reference listed.
The form provides space for up to four business references, with fields for business name, point-of-contact information, phone number, email address, and a description of services provided for each entry. This reference documentation serves as a critical evaluation tool for the state to assess bidder qualifications and experience relevant to the procurement. The revised form is dated December 6, 2022, and represents a standard requirement for respondents seeking to compete for this Texas state contracting opportunity.
