BUILDING CLIENT DATA

Fall Creek Bridge Federal Highway Administration Cascade Lakes Highway (Mile Post 26.6) 610 East 5th Street Deschutes County, Oregon Vancouver, WA 98661

PBS Engineering and Environmental Inc. (PBS) has performed a pre-removal asbestos inspection of the Fall Creek Bridge approximately located at mile post 26.6 on the Cascade Lakes Highway in Deschutes County, Oregon. The Asbestos Survey was conducted in general accordance with Oregon Administrative Rule (OAR) 340-248-0270 and PBS’ proposal dated July 28, 2016. Based on the information gathered during the site inspection and laboratory analysis results, this report contains the following information:

• A summary of asbestos-containing materials (ACM) and non asbestos-containing containing materials identified during the inspection, including a material description and the approximate location of each identified ACM,

• A sample inventory listing the sample number, approximate location, material description, and laboratory results,

• Laboratory analysis report and chain of custody documentation (Appendix A), and

• Inspector Certification (Appendix B)

SURVEY SCOPE

PBS endeavored to locate all accessible bridge components suspected to contain asbestos. Additional ACM may exist in concealed locations or in bridge components not sampled as part of this investigation. If additional suspect ACM are discovered during future bridge removal activities that are not identified in this report, testing should be performed prior to impact.

CERTIFICATION

PBS has conducted a physical inspection of the Fall Creek Bridge, compiled this report consistent with the survey scope, and certifies that the information is correct and accurate within the standards of professional quality and contractual obligations.

1.3 August 2016

PBS Project No: 80727.000

DATE SURVEYED BY ACTIVITY

August 10, 2016 Alex Fischer Asbestos Survey

STRUCTURE DATA:

The structure is a three span wood bridge with a concrete bridge deck and asphalt road surface.

Bridge Length: Approximately 56 Feet

ASBESTOS MATERIALS

No ACM were identified within the inspected portions of the Fall Creek Bridge. If suspect ACM are discovered during future bridge removal activities that are not identified in this report, testing should be performed prior to impact. Materials not sampled may contain asbestos and should be tested to verify asbestos content prior to impact through demolition, renovation, etc.

MATERIALS THAT TESTED NEGATIVE FOR ASBESTOS

The following materials tested negative based on ASHARA sampling minimums and testing by an NVLAP participating laboratory. Although no asbestos was detected, it is possible that further sampling could indicate asbestos content.

Material Location Concrete Concrete bridge deck Bridge Span Sealant Sealant applied between wooden bridge spans Expansion Pad Expansion pad applied between concrete bridge spans Tar Tar applied to wood bridge supports and asphalt road bed

Asbestos bulk samples obtained from the Fall Creek Bridge were collected by an EPA AHERA accredited inspector and analyzed using Polarized Light Microscopy (PLM) with dispersion staining. Samples were submitted under chain of custody to Lab/Cor in Portland, OR (NVLAP # 200741-0) for analysis. The laboratory analysis report is presented in Appendix A of this report.

Asbestos and Environmental Analysis

This laboratory participates in the National Voluntary Laboratory Accreditation Program (NVLAP).

Testing method is per 40 CFR 763 Subpart E, Appendix A, PLM.

•"NAD" is No Asbestos Detected.

•Asbestos consists of the following minerals: chrysotile, amosite, crocidolite, tremolite, actinolite, anthophyllite.

•Material binders, such as those found in vinyl floor tiles, may prevent the detection of small diameter asbestos fibers. A gravimetric preparation and point-count is recommended for such samples.

•Quantitative analysis by PLM point count or TEM may be recommended for samples testing at < or = to 1% asbestos.

•The following estimate of error for this method by visual estimation of asbestos percent are as follows:

•1% asbestos: 0-3% error, 5% asbestos: 1-9% error, 10% asbestos: 5-15% error, 20% asbestos: 10-30% error.

•This report pertains only to the samples listed on the report. Report considered valid only when signed by analyst
