REVISIONS ARE AS FOLLOWS:
BID SCHEDULE CLARIFICATION
Amendment A004 did not remove the following bid items:
• 5104-1000 Dynamic Pile Load Test (471+85 Bridge)
• 55104-1000 Dynamic Pile Load Test (588+50 Bridge)
Amendment A004 did revise the pay items so that they are no longer Contract Quantity pay items and they were removed from the Contract Quantity Pay Item list on page A-8.

ENCLOSURES
Pages: E-9 & F-71 Plans: G.7 & G.19
E-9
Special Contract Requirements Project: WA PFH 29-1(1), Middle Fork Snoqualmie River Road during use. This requirement may be waived if the source meets weed-free inspection requirements conducted by the Forest Service. Provide a written request to the CO and allow up to 30 days for the inspection to be conducted.
To arrange for a weed free inspection, contact Sarah Callagan, Forest Invasive Plant Specialist, at 425.785.6042 (phone), 425.783.0212 (fax) or to sacallaghan@fs.fed.us.
105.04 Storing and Handling Material. Delete the text of the second paragraph and substitute the following:
Use existing turnouts, parking areas, and portions of the right-of-way approved by the CO for staging or storing of materials such as culverts, geotextile fabric, temporary traffic control devices, and for equipment parking. The horse trailer parking area at the Middle Fork Trailhead is also available for staging and storing of materials. Coordinate with the CO at least two weeks prior to using the horse trailer parking area. Existing parking areas and turnouts, other than the Middle Fork Trailhead horse trailer parking area, must be available for public use during open road periods, unless otherwise approved by the CO.
Provide additional space as needed. Do not use private property for staging or storage without written permission of the owner or lessee. Furnish copies of all agreements. Comply with Subsection 105.02 with respect to any contractor-located staging or storage areas. Restore all Government-provided storage sites to their original condition.

F-71
Special Contract Requirements Project: WA PFH 29-1(1), Middle Fork Snoqualmie River Road the Contractor's method of operation. Furnish test piles at least 30 feet longer than the estimated length of production piles shown in the plans. Furnish production pile at least 10 feet longer than the lengths determined by the test piles.
551.06 Test Piles. Test pile locations are shown on the plans. Install test piles no later than May 1, 2014 at the 588+50 bridge. Do not order production piling until the test pile program is complete.
Use the results of the test pile to establish order lengths and driving criteria for the remaining production piles in the same abutment.
Excavate the ground at the site of each test pile to the elevation of the bottom of the footing before the pile is driven. Drive test piles with the same equipment as the production piles.
(a) Initial drive. Drive test piles to the estimated tip elevation shown in the plans. Determine the driven pile capacity of test piles by dynamic load test with signal matching during initial drive. Provide a preliminary dynamic load test report to the CO within 24 hours after the pile is driven to the estimated tip elevation. Within 24 hours after receipt of the initial report the CO will either require driving the test pile deeper with additional dynamic load test or indicate that the set up period for the restrike test has begun.
(b) Set up period. Do not drive any piling at the same abutment during the set up period.
Allow test piles to set up for at least 21 days before restrike.
Do not proceed with the restrike test with the second test pile on the other abutment at the same bridge until the CO has approved the preliminary restrike results. The set up period of the test pile at the other abutment may be extended up to an additional 21 days if the first restrike indicates the pile has not obtained the required nominal bearing resistance.
(c) Restrike. Warm the hammer before restrike begins by applying at least 20 blows to another pile that is at least 20 feet from the test pile. Perform a dynamic load test with signal matching from the first foot of penetration during the restrike. Provide a preliminary dynamic load test report to the CO within 24 hours after the restrike.
If the required nominal bearing resistance is not attained on restrike, drive a portion or all of the remaining test pile length as directed by the CO, and repeat the set up and restrike procedure as directed. Splice and continue driving until the required nominal bearing resistance is obtained.
Test piles to be used in the completed structure shall conform to the requirements for production piles. Remove test piles not incorporated in the completed structure to at least 2 feet below finished grade.
551.07 Driven Pile Capacity. Determine the compressive nominal bearing resistance of the test piles during driving by performing dynamic load tests with signal matching. Use the results of the test pile to establish the driving criteria for the remaining production piles in the same abutment.
Use the results of the dynamic load tests to establish the set up factor for the remaining production piles in each abutment, where the set up factor is equal to the nominal resistance at restrike divided by the nominal resistance during the initial drive determined by the dynamic load test. Use the set up factor to establish the required compressive nominal bearing resistance of production piling during initial drive that will achieve the required compressive nominal bearing resistance after set
