About this file
This is a reports inventory document for the West Virginia Department of Health and Human Resources (DHHR) PATH (Practices and Technology for Home) system, a comprehensive child welfare and social services case management platform. The document catalogs over 150 reports across multiple functional areas including child protective services, foster care, adoption, adult protective services, financial client accounts, IV-E claiming, court proceedings, and federal compliance reporting. The reports serve various audiences from direct service workers to executive leadership and federal agencies, with refresh frequencies ranging from real-time on-demand exports to quarterly snapshots. Reports are being transitioned from legacy FACTS and FREDI systems to new Cognos and in-application screen export functionality within the PATH platform, with many legacy reports undergoing consolidation, enhancement, or retirement based on 2020-2022 workgroup decisions.
The inventory identifies report status as retained, merged, deferred, or retired, with detailed notes on planned modifications, data sources, retention periods, and technical considerations. Several reports critical to federal compliance (AFCARS, NYTD) and state operations (IV-E determination, case workload management, safety planning) are flagged for retention with modifications to align with PATH data structures and business rules. Notable deferred items include all Child and Family Services Review (CFSR) reports pending federal redesignation from round 3 to round 4 indicators effective October 1, 2022. Numerous reports are undergoing consolidation to reduce redundancy while maintaining required functionality through enhanced filtering and drill-through capabilities. The document reflects ongoing collaboration between DHHR, Optum (the PATH contractor), and CGI regarding data warehouse design, batch processing functionality, and interface development to ensure seamless transition from legacy systems while meeting federal reporting requirements and state operational needs.

| PSS-ADM0010 | NCANDS Agency Summary Statistics | This report contains statewide totals reported on Section 2 of the NCANDS Agency File for screened out referrals and average response time. Data is broken out quarterly to show the federal fiscal year in progress. | Annual (FFY) content refreshed weekly | No planned archival without DHHR approval | Data | See 'PATH Social Service Cognos Reporting Roles and Entitlements' | Ad Hoc | Cognos | Report | CPS5090 | FFY Count | Proposed Description: This report shows the number of referrals for the Federal Fiscal Year. | R0488, R0489 | FFY Count | Merge | The PATH design of this report replaces the content of CPS5090 'FFY Count' and CPS5100 'NCANDS - Clients Screened Out'; there is no separate D043 Reports Inventory entry for CPS5100 as no sample was available but it was described as similar to CPS5090. In addition, summary information on this report was formerly sourced from the following legacy reports: CPS5220 'Drug Affected Infant' and No ID (RFP R0328/R0329) 'Time to First Contact Report (M-Force Reports Folder)'. The latter two legacy reports are not considered part of the CPS5090 and CPS5100 merge into PSS-ADM0010 because they are not replaced by PSS-ADM0010, they are retained separately in addition to PSS-ADM0010. (PS 2/3/22) |
Per CW-131, merge key statistics into new version called 'NCANDS Agency Summary Statistics'; see WAI and mtg notes for content. (PS 6/11/20)

CGI App: Partial Mike J Comment: The legacy system has a very robust query tool that sits in front of the IV-E determination module that providers the upfront GUI to allow the export of a focused and meaningful result set. How refined the query can get will in great part dictate how useful this tool is. There is a lot of flexibility to manipulate IV-E data in Cognos, but to make that trade the warehouse data would need to be changed to a daily refresh of the determination data. This needs further discussion with the state and CGI

Consider related M&O/Admin reports on the inventory and leverage the mature FACTS process in this analysis. (Mike J)Note: This is an OMB requirement for Single Agency Audit. It demonstrates system access and security roles. Per Traci, this is both a list of users with permissions and also a process for supervisors to review this information. Today in FACTS the security/access profile can be printed and signed off.
Per Mike S, each user is associated with their security/access profiles and can be viewed on a screen. Their profile history is also available. PATH does not have a sign-off indicator. (PS 7/16/20) Excel Export: Y Print:
CGI App: Not Exist Mike J Comment: This is not a CGI child welfare specific functionality. Recommend retire and address with whatever tool PATH will incorporate to administer and manage the required annual security and access auditing. Out of CW scope.

03/02/2021: Per meeting with Montrose team (Kevin Bell): OFS0594, will be delivered by Client Account Log screen in PATH. Will not be delivered as COGNOS report. Subledger current balance will not appear in the screen due to missing running balance functionality. Reason for Withdrawal appears in OFS0594 but not in Client Account Log screen. Kevin Bell confirmed that reason for Withdrawal will not appear in Client Account log screen but when a user exports client account log from client account log screen then in the excel export Reason for Withdrawal will appear.
Per State review, OFS0247 should be retained and not merged with OFS0594 and OFS251. Merge of OFS0594 and OFS251 may be possible depending on design. Review with State needed. Include OFS0247 in merge analysis. Include OFS0244 in this analysis as well (currently doesn’t show a true beginning balance for clients starting mid-month).(Tammy L, Stacey J, Jacquelin Hoppe). Lake Furney to send info. Note that a screen solution would reflect near real time information while a report would at best be refreshed daily. Other discussion: Tammy L – this was discussed. Needs to include all subledger activity including closures and interest. Subledger closures work differently in PATH vs FACTS. Show all subledgers for a given client.
The report is needed in Cognos to include multiple clients.
PATH will also have a Client Account Log screen with robust filtering which could be the starting point for a screen export that is specific to the current client. There is also a view of activity across clients.

Per CW-135: Action Item: Review PATH functionality around these screens to determine whether a Cognos report is still needed as well (related to OFS0594, OFS0251).
| Financial | Client Accounts (CAC) | RPT0530 | |||||||||||||
