About this file
This is a Diversity Practices Questionnaire issued by the Office of Contractor and Supplier Diversity in New York State, designed to assess prospective contractors' commitment to supplier diversity initiatives. The questionnaire requires respondents to provide comprehensive information regarding their diversity programs and practices, including details about designated diversity leadership, financial commitments to minority and women-owned business enterprises (MWBEs), technical training initiatives, mentor-protégé program participation, quantitative utilization goals, formal supplier diversity programs, and planned subcontracting arrangements with certified MWBEs. Respondents must complete all sections under penalty of perjury and provide supporting documentation where applicable, including a Utilization Plan if subcontracting with MWBEs is anticipated.
The questionnaire addresses eight primary areas of inquiry, requiring companies to disclose the percentage of prior fiscal year revenues and overhead expenses directed to New York State certified MWBEs, document any technical training programs offered to minority and women-owned enterprises, identify participation in government-approved mentor-protégé programs, and describe specific quantitative goals established for non-government procurement activities. All information is subject to audit, with fraudulent statements exposing respondents to criminal prosecution and debarment. The related Centers for Advanced Technology contract opportunity in New York establishes MWBE contracting goals of 30 percent total, with a 6 percent set-aside specifically for Service-Disabled Veteran-Owned Businesses, reflecting the state's commitment to inclusive economic development and supplier diversity in technology-focused initiatives.
