About this file

This is a Disqualification Disclosure form required by the State of California under Public Contract Code Section 10162, applicable to the Pasadena City Attorney/City Prosecutor's Office Prop 47 HOME Diversion Program Services contract opportunity. The form serves as a mandatory questionnaire that prospective bidders must complete under penalty of perjury to establish their eligibility to bid on this state and local government project. Bidders are required to disclose whether they, any company officers, or any employees with proprietary interests in the bidding entity have ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.

The disclosure form requires bidders to indicate either "Yes" or "No" in response to the disqualification question, with instructions that any affirmative response must be accompanied by a detailed explanation of the circumstances and supporting documentation attached as an exhibit. This form is a standard compliance requirement designed to ensure that only qualified entities without histories of regulatory violations or legal infractions participate in the procurement process for the HOME Diversion Program, which is funded through the California Board of State and Community Corrections Prop 47 Cohort 5 grant and involves providing diversion services and wraparound support to unhoused individuals with mental health and substance use disorders.
