About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for the Fair Oaks Substation Professional Design Services project. The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer of the bidder, or any employee with a proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. Bidders must indicate their response by checking either "Yes" or "No," with instructions to explain circumstances and attach supporting documentation if the answer is affirmative.

The Fair Oaks Substation project seeks qualified engineering firms to provide design services addressing equipment obsolescence and supporting the Public Works Power program's conversion from 4-kV to 17-kV distribution capacity. The project is structured in two phases with a desired Phase One in-service date of June 2029, and the engineering services include final design, bid assistance, design-build bridging documents, and construction-phase engineering support. This disclosure form is a standard compliance requirement for all bidders on this state and local contract opportunity and must be completed prior to proposal submission.
