About this file

This is a Site-Specific Safety Plan document for Breinholt Contracting Co., Inc., a construction contractor based in Phoenix, Arizona. The plan governs safety protocols for a demolition project located at 3345 and 3331 W. Monterosa Street in Phoenix, Arizona (Job #13362). Key personnel include Project Manager Jon Garner, Project Superintendents Art Reyes and Sam Breinholt, and Project Safety Coordinator Yvonne Lockhart. The document establishes safety requirements in compliance with State of Arizona and Federal OSHA standards, the Breinholt Safety Manual, and project owner requirements. The plan applies to all Breinholt employees, subcontractors, and consultants working on the project.

The project scope involves demolition of existing residential structures, including concrete slabs, footings, walkways, driveways, site walls, fencing, miscellaneous debris, trees, and shrubs. The safety plan details comprehensive requirements including personal protective equipment (hard hats, safety glasses, hearing protection, high-visibility clothing), hazard communication procedures, tool safety specifications, fire prevention measures, environmental controls, and emergency response protocols. The plan establishes a hierarchical enforcement structure with progressive disciplinary action for safety violations, ranging from first-time warnings to employee removal for repeated infractions. Additional requirements include daily safety inspections, toolbox safety meetings, MSDS documentation, first aid provisions, and designated emergency medical facilities (St. Joseph Hospital). The document emphasizes that safety cannot be compromised for scheduling deadlines and that all personnel are responsible for maintaining a safe work environment.
